老年勃起功能障碍与下尿路症状相关性的研究

目的:了解老年下尿路症状(LUTS)患者的勃起功能障碍(ED)患病率及不同程度ED患者的LUTS程度,评估ED与LUTS之间的相关性。方法:分析136例老年LUTS患者ED患病率与对照组进行比较;对其中的112例老年ED患者按照国际前列腺症状评分(IPSS)分为轻度、中度、重度LUTS 3组,比较3组间国际勃起功能指数-5(IIEF-5)评分和阴茎海绵体动脉的收缩期最大血流速率(PSV)和舒张末期血流速率(EDV)。结果:136例LUTS患者ED患病率与对照组比较有统计学意义(P<0.01),3组LUTS组间随着LUTS程度逐渐加重,IIEF-5评分、PSV呈下降趋势,其中轻度LUTS组与中度、重度LUTS组间比较差异均有统计学意义(P<0.05)。3组间EDV比较差异无统计学意义(P>0.05)。结论:老年LUTS患者ED患病率高于对照组;老年ED患者性生活的质量与LUTS程度相关,对阴茎血流动力学的影响可能是LUTS影响ED程度的途径。

Correlation of Erectile Dysfunction with Lower Urinary Tract Symptoms in Elderly Men

Objective:To investigate the incidence of erectile dysfunction(ED) in lower urinary tract symptoms(LUTS) and degree of LUTS in people with ED in elderly men;evaluate the correlation between ED and LUTS.Methods:Analysis prevalence of ED in 136 cases elderly men with LUTS compared with the control group without LUTS.Divid total 112 patients with ED into mild group,moderate group and severe group according to international prostate symptom score(IPSS).Every patient took a 5 item version of the international index of erectile function(IIEF-5) test.The peak systolic velocity(PSV) and end diastolic velocity(EDV) of cavernosal artery were measured by color doppler ultrasonography.Then the results were statistically analyzed.Results:There was significant differences of the incidence of erectile dysfunction(ED) in lower urinary tract symptoms(LUTS) and control team(without LUTS),the means of IIEF-5 scores and PSVs decreased along with the progression of LUTS and significant differences were found when mild group compared with moderate and severe group(P<0.05).There was no significant difference of EDV among three groups(P>0.05).Conclusion:The incidence of ED is high in men with LUTS compare with control team.The sexual life quality in elderly men with ED decreases along with the progression of LUTS and hemodynamic effect may be the cause.
