腹腔镜联合纤维胆道镜胆总管探查取石术22 例

目的探讨腹腔镜下胆总管探查取石术的适应证、方法及优点。方法回顾性分析2009年10月至2011年4月空军总医院肝胆外科22例腹腔镜胆总管探查术(LCBDE)患者的临床资料。结果 21例成功行LCBDE,1例术中发现为Mirizzi综合征中转开腹,手术时间(125±45)min,术后平均住院时间(5.5±2.0)d,T管引流2～4周拔管。22例均无胆瘘、残余结石,随访2～18个月无残余结石及胆管狭窄。结论 LCBDE具有微创、并发症低、住院时间短等优点,有较高的临床应用价值。

Analysis of laparoscopic bile duct exploration combined with fibercholedochoscope in 22 cases

Objective To discuss the advantage,method,and indication of laparoscopic common bile duct exploration（LCBDE） and calculus removed.Methods The clinical data of 22 cases with common bile duct calculi,who came from air force general hospital from October 2009 to April 2011.Results Twenty one cases of common bile duct gallstone were operated with LCBDE successfully,1 case was performed with open laparotomy because finding Mirizzi syndrome.Average operation time was（125±45） minutes,the average stay was（5.5±2.0） days,T tube drain for 2-4 weeks.No cases had bile leakage or residual stones.All cases were followed-up for 2-18 months,no bile duct stricture and residual stone was found.Conclusion LCBDE has the advantage of micro-injury,less complication and short length of stay.It is significant for clinical application.
