颞下颌关节继发性滑膜炎保守治疗与关节腔注射治疗的对比观察

摘    要:
目的探讨颞下颌关节继发性滑膜炎的治疗方法。方法选择患颞下颌关节滑膜炎的患者共56例,根据病因分为单纯组、关节盘移位组,分别采用理疗、药物治疗等保守方法和关节腔注射进行治疗,对比两种方法的治疗效果。结果单纯组中保守治疗效果与关节腔注射的无显著差别,关节盘移位组中关节腔注射优于保守治疗。结论对颞下颌关节滑膜炎要分清病因进行治疗,单纯性滑膜炎保守治疗或关节腔注射治疗效果均好,但保守治疗简便易行,几乎无创,更适宜于治疗单纯性滑膜炎;盘移位导致的滑膜炎宜采用关节腔注射治疗。

Abstract:
Objective To find out a better therapy for temporomandibular joint synovitis. Methods 56 temporomandibular joint synovitis patients were divided into two groups:group A (24 patients who were diagnosed as disc displacement or osteoarthrosis) and group B (32 patients without disc displacement or osteoarthrosis). Each group was divided two groups again, and the patients received intraarticular injection or conservative therapy respectively. Curative effects were observed. Results After three months, 56 patients were followed up. In group A, patients who received intraarticular injection had better curative effects (P < 0. 05). In group B, there was not significantly different effect for two methods(P > 0. 05). Conclusions When we treat synovitis patient with disc displacement or osteoarthrosis,it is a primary way to use intraarticular injection. But for other patients with synovitis, it is a more reasonable way to choose conservative therapy.
