异位妊娠未破型孕囊与妊娠卵巢黄体小囊肿的二维超声对比分析

目的利用二维超声对异位妊娠未破型孕囊与妊娠卵巢黄体小囊肿进行对比分析,以提高异位妊娠的早期检出率。方法对32例输卵管妊娠未破型孕囊与40例妊娠卵巢黄体小囊肿的二维超声图像作回顾性对比分析。结果输卵管妊娠未破孕囊有＂输卵管环＂回声,而无1例妊娠黄体小囊肿有此特征;90%的妊娠黄体小囊肿周围可见卵泡回声,而无1例异位妊娠孕囊周围可见卵泡回声;90%的妊娠黄体小囊肿伴有子宫内膜蜕膜样改变。结论通过两组资料的回顾性对比分析,二维超声显示的卵巢附近及子宫旁一侧＂输卵管环＂图像是扩张输卵管内孕囊着床的直接征象,也是早期宫外孕未破型孕囊诊断的重要依据。结合子宫内膜回声和临床症状,二维超声也不难鉴别输卵管妊娠未破孕囊与妊娠黄体小囊肿的诊断。

Comparative analysis between non-ruptured gestation sacs of ectopic pregnancy and small corpus luteum cysts of early pregnancy with 2D ultrasonography

Objective To compare the sonographic echogenicity of non-ruptured gestation sacs（NRGS） of ectopic pregnancy with small corpus luteum cysts（SCLC） of early pregnancy and to improve the detective rate of early ectopic pregnancy.Methods The sonographic echogenicity of NRGSs in 32 women with tubal pregnancy was retrospectively analyzed and compared with that of SCLCs in 40 women with early pregnancy.Results The echogenicity of tubal ring was only found in NRGSs,which was invisible in any of SCLCs.The follicular echogenicity was found in most SCLC circumferences（90%）,which was invisible in any of NRGSs.The most SCLCs（90%） associated with decidua-like change of the endometrium.Conclusion Sonographic comparison of the both groups show that the ＂tubal ring＂ signs visible in ovarian neighbourhood or uterine side are direct signs for identifying the intrafallopian gestation and are also important evidence for early diagnosing ectopic pregnancy.The integration of the endometrial echogenicity revealed by 2D ultrasonography and clinical symptoms is not difficult to differentiate NRGS of tubal pregnancy from SCLC of early pregnancy.
