19例乳腺原位癌诊治及免疫组化分析

目的：探讨乳腺原位癌的诊断和治疗，分析其免疫组化特点。方法：回顾性分析我院2003年7月-2006年7月间诊治的19例乳腺原位癌的临床诊断及治疗，用免疫组化方法测定各抗体的表达率并进行分析。结果：19例乳腺原位癌占同期乳腺癌病例的6．6％，平均就诊时间12．4个月。其中18例乳腺原位癌的雌激素受体（ER）、孕激素受体（PR）、p53基因和人类表皮生长因子受体2（c-erbB-2）阳性表达率分别为66．7％、77．8％、38．9％、27．8％。结论：p53、c—erBb-2以及ER、PR表达状况有助于判断乳腺原位癌预后和选择适宜的治疗方案。乳腺原位癌手术预后较好，保乳术成为术式的选择之一。

Diagnosis, Treatment and Immunohistochemical Analysis of 19 Cases of Breast Carcinoma in Situ

Objective:To investigate the diagnosis and treatment of breast cancinoma in situ and analyze the immunohistochemical characteristic. Methods:The clinical data of 19 cases of breast carcinoma in situ from July, 2003 to July, 2006 were retrospectively reviewed and the immunohistochemical results were analyzed. Results:The 19 cases of breast carcinoma in situ accounted for 6.6% of all breast cancer cases for the same period. The patients were female with an average age of 49.9. All cases were discovered when patients came to treatment. The average time from the appearance to diagnosis was 12.4 months. Among 18 cases, the expression rates of ER, PR, p53 and c-erbB-2 were 66.7%, 77.8%, 38.9% and 27.8% respectively. Conclusion:The expression of p53, c-erbB-2, ER and PR is helpful to determine the prognosis and treatment of breast carcinoma in situ. The prognosis of breast carcinoma in situ is comparatively good after surgery. Breast-conserving treatment is an available choice for breast cancer.
