幽门螺杆菌感染与Barrett食管病变的相关性研究

目的 探讨除幽门螺杆菌(Hp)感染和Barrett食管病变的关系.方法 收集168例内镜检查患者的病历资料,其中Barrett食管患者86例(Barrett食管组),胃食管反流病(GRRD)患者51例(GERD组),食管黏膜的正常患者31例(对照组),采用快速尿素酶法和Warthin Starry银染两种染色方法检测各组患者Hp感染状态.结果 对照组、GERD组和Barrett食管组Hp感染率分别为0、13.7%(7/51)和37.21%(32/86),Barrett食管组Hp感染率明显高于对照组和GERD组(X2=15.87,P<0.01;X2=10.94,P<0.01);Barrett食管中、重度炎症组Hp感染率明显高于轻度炎症组(X2=15.96,P<0.01);特殊肠化组Barrett食管Hp感染率略低于胃底和贲门型组,差异无统计学意义(P>0.05).结论 Hp感染与Barrett食管病变相关,Hp感染加重Barrett食管病变炎症程度.

Abstract：

Objective To investigate the relation between Barrett esophagus(Barrett)and helicobacter pylori(Hp)infection.Methods One hundred and sixty eight patients were enrolled,including those affected by Barrett (n=86),gastreesophageal reflux disease(GERD n=51)and control group(n=31).Results The prevalence ofrate of Hp infection in Barrett was significantly higher than that in GERD control groups(X2=15.87.P<0.01;X2=10.94,P<0.01).The rate of Hp infection in the moderate and severe grade esophagitis was significantly higherthan that in the mild group(P<0.01).r11lle rate of Hp infection in the Barrett with intestinal metaplasia was lowerthan that in the Barrett group,but no significant difference was found.Conclusion Hp infection plays an important role in Barrett and its inflammation grades.
