| 摘 要： | 目的:比较全麻联合硬膜外麻醉与单纯全麻两种麻醉方式对老年腹腔镜胆囊切除术(LC)患者循环功能和麻醉效果的影响,探讨两者的临床应用疗效及安全性。方法:60例有胆囊结石需腹腔镜手术老年患者随机分为全麻联合硬膜外麻醉组(A组,n=30)与单纯全身麻醉组(B组,n=30),监测麻醉起效时间,麻醉前后、气腹后10min、30min时SBP/DBP、HR、SpO2、术后疼痛(VAS评分)及术后清醒时间,评价麻醉效果及安全性。结果:B组在麻醉后和气腹后SBP/DBP及HR均增高,并持续至气腹后30min(P<0.05),A组各指标无明显差异(P>0.05),两组均无术后严重并发症。结论:全麻复合硬膜外麻醉在腹腔镜手术中更能提供可靠稳定的血流动力学的变化,老年患者采用全麻复合硬膜外麻醉更加安全。
