输尿管镜气压弹道碎石术治疗输尿管结石472例分析

目的 总结输尿管镜气压弹道碎石术治疗输尿管结石的疗效、经验及操作技巧.方法 回顾性分析472例输尿管镜气压弹道碎石术治疗输尿管结石的患者资料.结果 碎石成功率91.10%(430/472),33例结石移位至肾盂而行体外震波碎石术,操作失败9例.输尿管损伤16例,其中黏膜下假道形成8例,穿孔5例,部分断裂3例.术后1个月结石排净414例,结石排净率87.71%.结论 输尿管镜气压弹道碎石术创伤小,成功率高,掌握好手术适应证及术中操作技巧,辅以体外震波碎石及其他治疗手段,可最大限度减少并发症,提高结石排净率.

Treatment of ureteral calculi with ureteroscopic pneumatic lithotripsy

Objective To summarize the clinical experience of the treatment of ureteral calculi with ureteroscopic pneumatic lithotripsy.Methods 472 cases of uteteral calculi were analysed.Results The overall comminution rate was 91.10%(430cases).The operation failed in 33 cases due to the shift of the stones to the renal pelvis.16 cases complicated ureter injured including perforation,lancinate and synthentic.414(87.71%)cases were stone free within 1 month.Conclusion Ureteroscopic pneumatic lithotripsy is an effective method for treating ureteral calculi,with advantage of high effective,safety,minimal invasion and simplicity of performance.To master operating indication strictly,raise operating technique and combined use of ESWL could raise success ratio.
