血透患者细胞免疫功能的变化

目的；探讨血透对患TI林巴细胞免疫功能的影响。方法：观察19例尿 毒症血透患透析前，透析5h,24h,48h,72h后外周血中T淋巴细胞亚群和白细胞，淋巴细胞计数，进行各时点指标与术前比较的方差分析，组间两两比较配对t检验，结果：患透析5h,24h的血白细胞，淋巴细胞显低于血透前水平，透析48h,72h后接近透析前水平，两差异无显性，透析前T淋巴细胞CD3^ ,CD4^ ,CD57^ 水平低于正常对照组，CD8^ 高于正常对照组，CD4^ /CD8^ =0.86,透析48h后CD3^ ,CD4^ ,CD57^ 较透析前明显增高CD4^ /CD8^ =1.26，CD8＋水平在透析前后无显改变，结论：血液透析对机体细胞免疫功能影响是多方面的，一方面由于血透后血浆中尿毒症毒素部分清除使T细胞免疫功能可获暂时性改善，同时由于血液透析过程对白细胞趋化，聚集和粘附作用的影响，外周血白细胞数量下降致使机体的免疫就答作用降低，应当重视对血透患免疫机能调节。

The change of the immune function of cells in hemodialysis patients

Objective:To study the effects of hemodialysis in immune function of T-lympho-cells of patients.Methods:We observe 19 hemodialysis patients of uraemia and stud the conunt of T-lympho-cells subgroups,white cells and lympho-cells in exterior circulation of blood before trans-analysis,and after 5 hour,24 hour,48 hour and 72 hour respectively,and investigate variance analysis of index in every hour and count and compare before operate and t-test of pairing of comparing in twos between groups.Results:The white cells and lympho-cells of trans-analysis 5 hour and 24 hour of patients are remarkably lower than level before trans-analysis,but they are no different almost after trans-analysis 48 hour and 72 hour and before trans-analysis.Before trans-analysis,level of T-lymph-cell CD++-3?CD++-4?CD++-{57} is lower than normal contrapositive group but CD++-3 is higher than normal contrapositive group,CD++-4/CD++-8=0.86,after trans-analysis 48 hour,CD++-3?CD++-4?CD++-{57} heighten apparently,CD++-4/CD++-8=1.26 but the level CD++-8 do not vary almost from beginning of trans-analysis to end.Conclusion:It is in many aspect that hemodialysis affect the immune function of cells of organinsm.First because the immune function of T-cells can obtain improvement of temporality by eliminating partial poison of uraemia after hemodialysis in blood plasma,and meanwhile because the process of hemodialysis affect action of changing trend,clollection and adhering of white cells,the decreasing of the number of white cells of exterior circulation of blood bring the droping of action of replying of immunity of organism.So it must be remarked adjusting the immune function of the patients of hemodialysis.
