B超引导下臂丛神经阻滞麻醉的临床效果分析

目的探讨B超引导下臂丛神经阻滞麻醉用于上肢手术的麻醉效果及安全性。方法选取我院骨科120例拟行上肢手术患者,随机分为2组,A组采用臂丛神经阻滞盲探式操作,B组采用B超引导下的臂丛神经阻滞,两组均采用1%利多卡因+0.25%布比卡因合剂。结果 A组与B组相比,麻醉效果及安全性存在明显差异:A组麻醉效果优良率为83.3%。B组麻醉效果优良率为93.3%,两组比较差异有统计学意义(P<0.05)。麻醉过程中患者发生寒战、惊厥发生率及血管神经损伤、止血带反应等不良反应率B组明显少于A组,差异有统计学意义(P<0.05)。结论 B超引导下臂丛神经阻滞运用于较长时间的上肢手术是更为合适,提高了穿刺、置管成功率,增加了麻醉效果的优良率,减少了并发症的发生率。

Clinical Analysisof B Ultrasound-guided Brachial Plexus Anesthesia

Objective To observe the safety and efficacy of B uItrasound-guided brachiaI pIexus bIock anesthesia in upper Iimb operation.Methods 120 patients in our hospital undergoing upper Iimb operation with bracheaI pIexus bIock anesthesia were divided into two groups(60 cases each group).Group A use Brachial plexus block blind operation.Group B use B ultrasound-guided Brachial plexus anesthesia.The anesthetics were 1% Iidocaine mixed with 0.25%bupivacaine.Results Significant difference in group A compared to group B,the anesthetic effect and safety: A group of excellent and good rate was 83.3%,but the Group B anesthesia excellent rate was 93.3%,group B,the difference was statistically significance(P<0.05).Conclusion B ultrasound-guided brachial plexus block used in a long time the upper extremity surgery is more appropriate to improve the puncture catheter success rate,an increase of the anesthetic effect of the fine rate,reducing the incidence of complications.
