糖尿病合并肺部感染151例临床分析

目的分析糖尿病伴肺部感染的临床特点，探讨其治疗策略。方法对151例糖尿病伴肺部感染患者临床相关因素（性别、年龄、病程、血糖、并发症、痰菌等）和治疗效果进行回顾性分析。结果糖尿病伴肺部感染的发生率为26．35％，其发生与年龄、病程、血糖高低及同时存在的糖尿病其他并发症有密切关系（P〈0．05或0．01），而与性别无关；肺部感染类型以急性气管-支气管炎为主（占39．07％），其次为肺炎、慢性支气管炎、肺结核等。肺部感染以革兰阴性杆菌为主。151例中痊愈和显效106例（占70．19％），死亡19例（12．58％）。结论定期行胸部X线检查，控制血糖，预防和治疗糖尿病并发症，有效抗生素应用是防治本病的关键。

Clinical analysis for 151 cases of diabetes mellitus complicated by pulmonary infection

Objective To analyze the clinical characteristics of diabetes mellitus (DM) complicated by pulmonary infection, and to explore the treatment tactics. Methods A retrospective investigation including the related factors ( sex, age, course of disease, blood glucose, complications, bacteria in sputum) and therapeutic effect was performed in151 patients with DM complicated by pulmonary infection. Results The incidence of DM complicated by pulmonary infection was 26.35%, which was related closely to patient's age, course of disease, the levels of blood glucose and complications (P<0.05 or P<0.01) , but not related to sex. The pulmonary infections were mainly acute tracheobronchitis (39.07%) ,seconded by pneumonia, chronic bronchitis and pulmonary tuberculosis, etc. The infections were caused mainly by Gram-negative bacilli. Of all 151 patients, the cure rate and effective rate were 70.19% (106 cases), the mortality rate was 12.58% (19 cases). Conclusion The keys of protecting and treating diabetes mellitus complicated by pulmonary infection include regular chest X-ray examination, control of blood glucose, precaution , treatment of diabetic complications and application of effective antibiotics.
