电针郄募穴缓解肾绞痛及其伴随症状26例临床观察

目的 观察电针郄募穴缓解肾绞痛及其伴随症状的临床疗效.方法 将符合纳入标准的51例肾绞痛患者随机分为电针组26例和肌注组25例.电针组采用电针肾经膀胱经郄穴(水泉、金门)及相关募穴(京门、中极)30min;肌注组于患者患侧臀部一次性肌肉注射强痛定100mg.于治疗前后分别用简化McGill疼痛问卷(SF-MPQ)和视觉模拟评分法(VAS)进行疼痛评分,制定伴随症状量表进行伴随症状评分,观察两组临床总体疗效及疼痛改善情况.结果 两组总体临床疗效比较,电针组愈显率为76.92％,肌注组愈显率为44.00％,电针组明显优于肌注组(P＜0.01).在即时缓解疼痛方面,电针组愈显率为69.23％,高于肌注组的44.00％ (P＜0.05).电针组开始缓解疼痛所需时间短于肌注组(P＜0.05),而两组完全缓解所需时间比较差异无统计学意义(P＞0.05).两组伴随症状积分治疗后10、30、60、120min与治疗前比较差异均有统计学意义(P＜0.05或P＜0.01),两组治疗后60、120min与治疗后10min比较差异均有统计学意义(P＜0.05).结论 电针郄募穴治疗肾绞痛及其伴随症状见效快、作用强、24h内疼痛复发率低、操作简便,值得临床推广应用.

Effect of Electro-Acupuncture at the Cleft Points and Alarm Points on Renal Colic and A Accompained Symptoms in 26 Cases

Objective To observe the therapeutic effect of electro-acupuncture(EA)at the cleft points and alarm points on renal colic and accompanied symptoms.Methods Fifty one patients with renal colic were randomized into EA group of 26cases and intramuscular injection(IM)group of 25cases.EA group was given electro-acupuncture at the cleft points of kidney channelShuiquan(KIS),Jinmen(BL63)]and bladder channel and related alarm pointsJingmen(GB25),Zhongji(RN3)] for 30minutes.IM group was given intramuscular injection of 100mg of bucinperazine.A short form of the McGill Pain Questionnaire(SF-MPQ)and visual analog scale(VAS)were used to score the pain.Associated symptoms scale(ASS)was developed to score the associated symptoms.The change of clinical symptoms were observed in both groups and the syndrome effect was evaluated.Results The total effective rate of the EA group and IM group was 76.92% and 44.00% respectively,with significant difference between groups(P<0.01).The total effective rate in relieving pain of the EA group and IM group was 69.23%and 44.00%respectively,with significant difference between groups(P<0.05).Pain relief time required in the EA group was significantly shorter than that in the IM group(P<0.05).There was no significant difference between groups in complete remission time required(P>0.05).Comparing with before treatment,there were significant differences in relief of associated symptoms after 10min,30min,60min and 120min of treatment(P<0.05or P<0.01).Comparing with 10min of treatment,there were significant differences in relief of associated symptoms after 60min and 120min of treatment(P<0.05).Conclusion EA at the cleft points and alarm points for renal colic is effective,easy to operate,less toxicity,low cost and worthy of clinical application with a low recurrence rate within 24hours.
