超声骨刀与植骨联合治疗根分叉病变的研究

摘    要:目的:研究不同植骨材料和超声骨刀联合治疗第一磨牙根分叉病变骨下袋的治疗效果。方法:选择第一磨牙根分又病变骨下袋病例102例,随机分为4组:A组实施翻瓣术,应用超声骨刀,B、C组在此基础上分别植入羟基磷灰石骨粉和Bio—Oss骨粉,D组实施翻瓣术,植入Bio—Oss骨粉,在基线及术后1、6、12月检测牙周临床指标和根分又区骨密度。结果:四组病例术后临床指标均有所改善,与基线比较有显著性差异俨〈0.05),但组间比较无显著性差异(P〉0.05)。C组在骨密度增加方面较A、B、D组明显,而B、D组又较A组增加明显,均有统计学意义(P〈0.05)。结论:超声骨刀联合Bio—OS8骨粉植入治疗根分叉病变能获得更多的牙周再生和牙槽骨的重建。

Abstract:Objective:Evaluate the effect of combining different bone graft materials and piezosurgery for treating furca- tion involvement intrabony defects of the first molar clinically, nethod:102 first molars with furcation involvement intrabony defects class II -class III were divided into one of the four groups at random: using piezosurgery in flap surgery in group A, using artificial hydroxyapatite bone meal and piezosurgery in flap surgery in group B, using Bio-Oss and piezosurgery in flap surgery in group C,using Bio-Oss in flap surgery in group D. Clinical indexes and the bone mineral density were measured at the baseline and 1,6,12 months after surgery. Result:A significant decrease was showed in clinical indexes in the four groups compared to baseline(P 〈0.05) and no significant difference between groups(P 〉0.05). An obvious increase of the level of BMD was showed in group C at 6,12 months after surgery, which illustrated significant difference comparing to group A, B,D (P 〈0.05). And the increase was more significant in group B,D comparing to group A(P 〈0.05). Conclution:The application of combining different bone graft materials and piezosurgery for treating furcation involvement clinically could Obtain more regeration of periodontal membrane and alveolar bone.
