超声心动图评估完全型肺静脉异位引流合并肺动脉高压

摘    要:目的应用二维及多普勒超声心动图分析完全型肺静脉异位引流(TAPVC)患者的肺动脉压力,通过术前、术后1周、术后3个月的对比,总结TAPVC患者肺动脉高压(PH)的特点及相关因素,建立评估此类疾病肺动脉高压的超声心动图方法。方法选择65例TAPVC患儿,分别于术前、术后1周、术后3个月进行超声心动图检查。通过二维超声多切面观察其心脏结构及功能。连续多普勒测量三尖瓣反流峰值压差(PTR),肺静脉干最大流速大于1.5m/s为静脉干梗阻(PVO)。建立超声评估肺动脉高压的方法。结果 65例TAPVC患儿均有显著肺动脉高压[PTR=(65.30±23.33)mmHg],13例发现有PVO。62例患者完成矫治手术。术后1周60例患儿PTR较术前明显下降(P0.01)。1例心上型TAPVC术后PTR不下降,超声发现静脉干与左心房壁吻合口狭窄。1例患儿右心回缩不明显,3个月后肺动脉压力持续增高,考虑存在肺静脉分支狭窄。术后3个月另有2例患者静脉干与左心房吻合口狭窄。其余患者术后3个月PTR下降至正常或接近正常,11例患者三尖瓣反流消失。结论 TAPVC合并肺动脉高压经手术矫治后肺动脉压力均可下降至正常范围,肺静脉回流受阻会导致持续肺动肺高压。超声可动态测量TAPVC患者的肺循环压力变化情况,并能准确诊断PVO的存在。
