单肺移植治疗终末期肺气肿

目的 总结临床肺气肿单肺移植围手术期的处理结果与经验体会.方法 回顾性分析6例临床肺气肿单肺移植的处理方法.6例患者均接受单肺移植手术,其中右肺移植3例、左肺移植3例.术后采用FK506 霉酚酯酸 甲基强的松龙三联免疫抑制治疗.结果 所有患者均度过术后早期(>30d).4例患者观察到急性排斥反应,经甲强龙冲击治疗后渡过.3例患者术后因并发症再次手术,1例因术后胸腔出血而手术并最终痊愈,2例因保留的自体肺发生自发性气胸而手术并分别在肺移植术后第74天和77天死亡.最终4例肺移植受者出院并存活至今,目前生存质量良好.结论 单肺移植是治疗终末期肺移植的有效方法,术前应谨慎地选择恰当的受者,根据其双肺的具体情况确定恰当的手术方式.
