益气通冠汤对冠心病稳定性心绞痛(气滞血瘀证)患者中医证候积分、血液流变学及心电图的影响

目的:观察益气通冠汤治疗对冠心病稳定性心绞痛(气滞血瘀证)患者中医证候积分、血液流变学及心电图的影响。方法:将90例冠心病稳定性心绞痛(气滞血瘀证)患者随机分为观察组和对照组,对照组45例给予规范西医治疗,观察组45例在对照组基础上给予益气通冠汤治疗,两组疗程均为4周。比较两组中医症状疗效、心电图疗效及治疗前后主症状评分、血流流变学、心电图改变情况。结果:观察组中医症候、心电图总有效率分别为88. 89%、71. 11%高于对照组的71. 11%、48. 89%,差异有统计学意义(P<0. 05);观察组治疗后胸闷、胸痛评分及全血粘度高切、全血粘度低切、血小板聚集率、红细胞聚集指数均低于对照组,差异有统计学意义(P<0. 05);观察组治疗后PTF-V1≤-0. 04mm·s几率为26. 67%低于对照组的46. 67%(P<0. 05),ST段异常略低于对照组,但差异无统计学意义(P>0. 05)。结论:西药联合益气通冠汤治疗冠心病稳定性心绞痛(气滞血瘀证),可减低血液粘度,对心电图改善的效果更好。

Effect of Yiqi Tongguan Decoction on Traditional Chinese Medicine Syndrome Scores,Hemorheology and Electrocardiogram for Patients with Coronary Heart Disease and Stable Angina Pectoris(Qi Stagnation and Blood Stasis Syndrome)

Objective:To investigate the effect of Yiqi Tongguan Decoction on TCM syndrome scores,hemorheology and electrocardiogram for patients with coronary heart disease and stable angina pectoris(Qi stagnation and blood stasis syndrome).Methods:90 patients with coronary heart disease and stable angina pectoris(Qi stagnation and blood stasis syndrome) were randomly divided into observation group and controlled group.45 cases in the controlled group were given standard treatment of western medicine.On the basis,the observation group were treated with Yiqi Tongguan Decoction.The two groups were treated for4 weeks.The curative effect on TCM symptoms,the curative effect of electrocardiogram,changes of symptom scores,hemorheology and electrocardiogram before and after treatment were compared between the two groups.Results:The total effective rates of TCM symptoms and electrocardiogram in the observation group(88.89% vs 71.11%) were higher than those in the controlled group(71.11% vs 48.89%)(P<0.05).After treatment,scores of chest tightness and chest pain,whole blood high and low shear blood viscosity,platelet aggregation rate and erythrocyte aggregation index in the observation group were lower than those in the controlled group(P<0.05).After treatment,the probability of PTF-V1≤-0.04mm·s in the observation group was lower than that in the controlled group(26.67% vs 46.67%)(P<0.05),and the rate of abnormal ST segment was slightly lower than that in the controlled group(P>0.05).Conclusion:The application of western medicine combined with Yiqi Tongguan Decoction in treating coronary heart disease with stable angina pectoris(Qi stagnation and blood stasis syndrome) can reduce blood viscosity and improve the electrocardiogram.
