动脉插管介入治疗股骨头缺血坏死(附68例疗效观察)

目的　探讨应用导管经旋股内、外动脉 ,闭孔动脉灌注治疗股骨头缺血坏死 (ANFH)的临床疗效。方法　选自 2 0 0 0 - 12～ 2 0 0 4 - 0 3介入溶通治疗股骨头缺血坏死 6 8例 10 8髋 ,对治疗前后疼痛、关节活动度进行比较 ,观察分析临床疗效。结果　介入治疗后 6 8例患者临床症状明显改善 ,髋关节疼痛、功能障碍得到缓解或消失。治疗后X线复查坏死骨质均有不同程度的吸收 ,新骨形成及修复。结论　介入治疗能改善股骨头的血液循环 ,是股骨头缺血坏死的有效治疗方法 ,Ⅰ、Ⅱ期的疗效优于Ⅲ和Ⅳ期。

Therapeutic Analysis on Interventional Therapy for 68 Patients with Avascular Necrosis of the Femoral Heal

Objective To approach the therapeutic effect of the treatment of ANFH by applying catheter into circumflex internal and external femoral arteries and obturator artery. Methods 108 hips in 68 patients of ANFH were treated by intervention. The score of pain, activity of joints, the clinical effects were observed and analyzed. Results Following interventional therapy on 68 cases, there were marked improvement of clinical symptoms with relief or disappearance of pain and functional disorder in hip joint. Reexamination with X-ray after treatment showed reabsorption of necrotic bone in different degrees, formation and repair of new bone. Conclusion Interventional therapy for the treatment of femoral head could have an effective treatment method, and the effect of the treatment in the I, II stage was better than that in the III and IV stage.
