髋臼"T"形骨折的诊断和治疗

目的 :探讨髋臼“T”形骨折的诊断步骤和治疗方法。方法 :自 1994年 8月～2 0 0 3年 1月共治疗髋臼“T”形 (B2型 )骨折 2 4例 ,其中B2 1型 3例 ,B2 2型 13例 ,B2 3型6例 ,“T”形 后壁骨折 2例。结果 :随访 1～ 4年 ,平均 2年 ,结果手术解剖复位组关节功能表现优良者 14例 ,可 2例 ;复位欠佳组关节功能表现良者 1例 ,可 2例 ;不满意复位组关节功能表现可 1例 ,差1例。非手术治疗组关节功能表现可 1例 ,差 2例。结论 :B2 1型髋臼骨折可以采用保守治疗 ,而B2 2型、B2 3型和“T”形 后壁骨折一般采用手术治疗。

Diagnosis and treatment of acetabular T-shaped fractures

Objective: To explore diagnostic procedure and treatment of acetabular T shaped fractures. Method: From Aug. 1994 to Jan. 2003,24 cases of acetabular T shaped (B2 type) fractures were treated, including 3 cases of B2 1 type, 13 cases of B2 2 type, 6 cases of B2 3 type and 2 cases of T shaped fracture with posterior wall fragment. Result: All patients were followed up for 1 year to 4 years, averaged 2 years. Among operative patients, there were 14 cases joint function reached excellent or good and 2 cases reached fair in anatomical reduction group, there was 1 case joint function reached good and 2 cases fair in imperfect reduction group, there was 1 case joint function reached fair and 1 case reached poor in unsatisfactory reduction group. Among nonoperative group, there was 1 case joint function reached fair and 2 cases reached poor. Conclusion: B2 1 type acetabular fractures can be treated with nonoperative therapy, B2 2, B2 3 and T shaped fracture with posterior wall fragment should always be treated with operation.
