正常前列腺和精囊的磁共振弥散加权成像初步研究

摘    要:目的探讨正常中青年男性前列腺和精囊磁共振弥散加权成像的可行性,并比较b值不同时,前列腺和精囊ADC值的差异.方法使用EPI扫描序列,对15例23~42岁的正常男性盆腔行磁共振弥散加权成像检查,b值分别为 300、600、800、1 000 s/mm2,测量前列腺中央带和外周带、精囊、膀胱、闭孔内肌、盆腔脂肪、直肠内气体的ADC值.结果在四个不同的b值时,均获得了前列腺中央带和外周带、精囊、膀胱、闭孔内肌、盆腔脂肪、直肠内气体的ADC值,在前列腺中央带和外周带、精囊、膀胱、闭孔内肌、盆腔脂肪、直肠内气体的ADC值间均有显著性统计学差异(P<0.05);b值越小,ADC值越大(P<0.05).结论 EPI-DWI扫描序列可用于前列腺和精囊的检查,不同b值测出的前列腺中央带、外周带及精囊的ADC值有差异.

Abstract:ObjectiveTo evaluate whether there was any difference between the apparent diffusion coefficients (ADCs) calculated on different diffusion weighted MR sequences using different b values. MethodsDiffusion weighted echo planar imaging (EPI) sequences with different b values were performed with 15 male volunteers, ages ranging from 23 to 42. The examinees with no symptoms of prostate and seminal vesicle diseases were examined by conventional MR T1WI and T2WI scan. Then images were obtained with factor b of 0, 300, 600, 800 and 1 000 s/mm 2. The ADC values of seminal vesicle, the central zone and peripheral zone of prostate gland, bladder, gas in the rectum, pelvic fat and muscle were measured. Software SPSS 11.0 was used to compare the ADCs obtained from the different sequences. ResultsAcceptable images for ADC measurement were obtained in all cases of different sequences. There was statistically difference of the mean ADCs of seminal vesicle, the central zone and peripheral zone of prostate gland, bladder, gas in the rectum, pelvic fat and muscle ( P <0.05) among the different sequences. The ADC values obtained with lower b values were larger than that obtained with higher b values. ConclusionADCs of seminal vesicle and prostate gland measured with EPI DWI sequence are different depending on the different b values.
