床旁滤过输血不同滴速对白细胞去除效果的观察

为探讨床旁滤过输血不同滴速去除全血中 WBC的效果 ,随机将 6 0例输血病人分为 A、 B两组 ,分别以 35、 70 gtt/ m in滴速输血 ,检测血液滤过前后白细胞 (WBC)、红细胞 (RBC)、血红蛋白 (Hb)、血小板(PL T)变化及病人在输血中的临床反应。结果两组滤过后 WBC计数均显著减少 ,与滤过前比较 ,P<0 .0 1,差异有极显著性意义 :滤过后 A组 WBC平均含量明显低于 B组 ,两组比较 ,P<0 .0 1,差异有极显著性意义 ;滤过对 RBC、Hb无显著影响。提示床旁滤过输血能有效去除库存全血中的 WBC;减慢滴速可提高 WBC去除效果 ;对RBC、 Hb无影响。但对血小板减少病人仍应慎用

Effect of Different Dropping Velocity on WBC Elimination during Filtering Transfusion beside Bed

In order to explore the effect of different dropping velocity on WBC elimination of whole blood during transfusion beside bed, 60 cases of transfusion were randomly divided into group A (transfusion by 35 gtt/min) and group B (transfusion by 70 gtt/min). The changes in WBC, RBC, Hb and PLT before and after blood filtration were studied and the clinical response of the patients observed. The results showed that the amount of WBC after filtration was significantly decreased in both groups as compared with that before filtration ( P <0.01). The average count of WBC in the group A was obviously lower than in the group B ( P <0.01). No notable effect of filtration on RBC and Hb was observed. It was suggested that filtering blood infusion beside bed could effectively eliminate the WBC in the preserved blood. Slowing down the dropping velocity could improve the effect of WBC elimination.
