血清C反应蛋白浓度在慢性心力衰竭诊断中的意义

摘    要:
目的探讨慢性心衰(CHF)治疗前后血清C反应蛋白(CRP)浓度与左室射血分数(LVEF)的相关性,分析其在判断心脏实际功能和评价CHF病情中的双重作用。方法95例CHF患者根据NYHA分级分为3组:心功能Ⅱ级组,心功能Ⅲ级组,心功能Ⅳ级组。分别于治疗前和治疗后采用免疫散射比浊法检测血清CRP浓度,利用B超检测心脏各指标计算LVEF。结果与治疗前相比,各组治疗后血清CRP浓度均降低,LVEF均增高(P<0.01);组间比较显示,除治疗后心功能Ⅱ级组与Ⅲ级组之间血清CRP浓度差异不显著外(P>0.05),治疗前和治疗后,心功能越差,血清CRP浓度越高,LVEF越低(P<0.01);CHF患者血清CRP浓度与LVEF负相关(r=-0.63,P<0.05)。结论血清CRP浓度越高,则LVEF越小,心功能越差,测定血清CRP浓度对于反映CHF严重程度,监测CHF疗效具有一定意义。

Abstract:
tein(CRP) and th Objective To explore the correlation between the serum concentration of C - reactive proe left ventricular ejection fraction (LVEF) and also to explore the value of the serum concentration of CRP during the treatment of chronic heart failure(CHF). Methods The 95 CHF cases were divided into cardiac function Ⅱ , Ⅲ, Ⅳ groups according to NYHA classification. Before and after the patients were treated, the figures of their hearts were determined by B - Ultrasound to calculate the LVEFs and the serum concentration of CRPs were determined by immune scatter turbidimetry. Results After being treated, the concentration of serum CRPs in each group were decreased while the LVEFs were increased, compared with that of the corresponding group without being treated ( P 〈 0.01 ). Groups with poorer cardiac function showed an increase of the serum CRPs concentration and a decrease of the LVEFs both before and after being treated(P 〈0.01 ) ,except for the serum CRPs concentration between cardiac function Ⅱ and Ⅲ groups after being treated ( P 〉 0.05 ). The serum concentration of CRP was negative correlated with the LVEF ( r = - 0.63, P 〈 0.05 ). Conclusion The higher the serum CRP concentration is, the lower LVEF and the poorer cardiac function are indicated, so detecting the serum concentration of CRP has the value of reflecting cardiac function.
