| 摘 要： | 目的探讨散发性直肠癌临床、病理特征与微卫星不稳定性（Microsatellite instability，MSI）表达之间的关系。方法检测75例散发性直肠癌手术标本DNA中5个位点的MSI表达，分析其表达与不同临床和病理特症之间的关系。结果正常结／直肠组织中无MSI阳性位点的表达。直肠癌组织中MSI-H（≥2个位点出现阳性）表达略为20．O％；MSI表达与性别、发现的淋巴结个数及阳性淋巴结个数、远处转移无明显关系；位于直肠上部的癌肿MSI高于位于下部者（P〈O．05）；位于直肠后半部的癌肿MSI高于位于直肠前半部者，但两者之间没有统计学差异；癌肿的左右、上下径，浸润深度与MSFH表达成反向关系；虽然有MSFH表达在患者年龄、癌肿在直肠的不同象限、非连续性扩散方面有增加，但无统计学差异。结论MSI表达对直肠癌预后的判定有一定的辅助价值，可作为TNM分期的一个补充，亦可作为判定高危直肠癌的参考指标之一，但与常规病理指标相比还有一些无法解释的矛盾现象，还需行前瞻性、随机性、大样本深入研究。
