陕西省白塞氏病81例临床分析

目的了解陕西省白塞氏病的临床特点。方法对符合ISGB诊断标准的81例白塞氏病的临床资料进行回顾性分析,并与中国其他地区以及其他国家研究结果进行比较。结果 81例白塞氏病患者,男:女为1.1:1;平均发病年龄(33.3±14.6)岁;平均诊断年龄(36.7±14.5)岁,从出现症状到诊断的时间为(4.9±8.3)年。71.6%的患者以反复发作的口腔溃疡为首发症状,整个病程中口腔溃疡的发生率为97.5%,皮肤病变为72.8%,外阴溃疡为61.7%,针刺试验阳性为45.7%。眼病者为33.3%,关节症状者占22.2%,发热为19.8%,血管损害为17.3%,消化道病变为7.4%,肾脏损害为3.7%,附睾炎为2.5%;男性毛囊炎或痤疮样皮损较女性高(P<0.05),血管损害高于女性(P<0.05),但女性关节损害高于男性(P<0.05)。结论白塞氏病的临床特点具有地域和性别分布差异,陕西省白塞氏病血管病变发生率高,关节病变发生率低。

Clinical feature of 81 patients with Behcet's disease in Shaanxi province

Objective To assess clinical features of Behcet’s disease(BD)in Shaanxi province.Methods Clinical data of 81 cases fulfilled the International Study Group for Behcet’s disease diagnositic criteria were retrospectively analyzed and compared with previous reports.Results The male/female patient ratio was 1.1:1.The mean age at onset was 33.3±14.6 years and the mean age of diagnosis was 36.7±14.5 years.Delay between onset of symptoms and diagnosis of BD was 4.9±8.3 years.Recurrent aphthous ulceration(71.6%)were the commonest onset-presentations of the disease.Throughout the study period,the commonest manifestation was oral ulcers(97.5%),followed by skin lesion(72.8%),genital ulcers(61.7%),the pathergy skin test(45.7%),ocular lesion(33.3%),articular involvement(22.2%),fever(19.8%),vascular lesion(17.3%),gastrointestinal involvement(7.4%),renal involvement(3.7%)and epididymitis(2.5%).The frequency of folliculitis or papulopustular and vascular lesion were significantly higher in males(P<0.05),while articular involvement was significantly higher in females(P<0.05).Conclusion The clinical features of BD showed geographical and gender difference,BD in Shaanxi province showed higher incidence of vascular,but lower incidence of articular involvement.
