高渗氯化钠溶液对水钠调节激素的影响

摘    要:比较30例硬膜外阻滞择期手术患者,输入7.5%HS和5%GS后血浆ADH、PRA、AⅡ及ALD变化。输入 HS后 10分钟 ADH增加 33%,AⅡ、ALD分别下降 37. 4%和 16. 5%,60分钟时 ADH仍维持在高水平,AⅡ、ALD亦未完全恢复。输入GS后 ADH有增加趋势,但无统计学意义,A Ⅱ于 10分钟时轻度降低,60分钟恢复,ALD逐渐增加,60分钟最明显(增加 46. 2%)。提示 HS可刺激 ADH释放,抑制RAAS活性,激素的变化对HS改善肾功能可能具有重要介导作用。

Abstract:The effects of hypertonic saline solution (HS) on antidiuretic hormone (ADH), plasma renin activity (PRA),anglotensin II (AII),and aldosterone (ALD) were evaluated, 30 patients scheduled for elective operations were infused with 7. 5 AHS (n = 20 ) or 5 % dextrose (n = 10) 4 ml/kg 20 min before epidural anesthesia. In HS group,the ADH increased about 33%,All and ALD decreased 37. 4%, 16. 5% respectively. In dextrose group, ADH increased but statistically not significant , All decreased transiently and ALD increased about 46. 2%. It is suggested that HS may stimulate the secretion of ADH and depress the activity of RAAS. These hormonal changes may play an important role for improving the renal function.
