60岁以上老年人腹腔镜直肠癌根治术近期疗效评价

目的探讨60岁以上老年人腹腔镜直肠癌根治术的安全性和可行性。方法对行直肠癌根治术的92例60岁以上老年患者的临床资料进行回顾性分析。根据手术方式分为腹腔镜组（36例）和开腹组（56例），对比分析2组手术情况、术后恢复情况和术后并发症。结果2组均无围术期死亡病例。腹腔镜组术中出血量、切口长度、排气时间、住院时间、手术并发症发生率与开腹组的差异均有统计学意义（P〈0．05），手术时间及淋巴结清扫数量差异无统计学意义（P〉0．05）。结论60岁以上老年人腹腔镜直肠癌根治术创伤小，术后恢复快，近期疗效好，是安全可行的手术方式。

A clinical study of laparoscopic rectal surgery for patients older than 60 years with rectal neoplasms

Objective To evaluate the feasibility and efficacy of laparoscopy and open radical resection for patients older than 60 years with rectal neoplasms. Methods A total of 92 patients aged older than 60 years who had undergone radical resection for rectal neoplasms between January 2011 and December 2013 at our hospital. The patients were assigned to laparoscopic group （n = 36） and open group （ n = 56）. The operation safety, postoperative recovery, and post- operative complications were compared between the two operative styles. Results No surgery - related death occurred in the two group. The blood loss, length of the incision, returning to bowel function, hospital stay and operative complica- tions for laparoscopic group were less than open group （ P 〈 0.05 ）. The operating time and the mean number of harvested lymph nodes had no significant difference between the two groups （ P 〉 0.05 ）. Conclusions The outcomes of this study suggest that laparoscopic radical resection for patients older than 60 years is safe and feasible, with its good short - time result and advantage of minimal invasiveness.
