胱抑素C和血小板活化因子在早期糖尿病肾病诊治中的应用价值

摘    要:目的探讨胱抑素C(CysC)和血小板活化因子(PAF)在早期糖尿病肾病(DN)诊治中的应用价值。方法测定30例糖尿病正常尿蛋白患者和28例糖尿病微量尿蛋白患者及15例正常对照组血清PAF及血清CysC的水平。结果CysC及PAF水平:糖尿病微量尿蛋白患者组[(1.79±0.92)mg/L,(92.38±30.25)μg/L]与糖尿病正常尿蛋白患者组[(1.42±0.41)mg/L,(67.57±19.46)μg/L]均较正常对照组[(0.74±0.21)mg/L,(50.71±15.43)μg/L]明显增高,差异有统计学意义(P〈0.01),糖尿病微量尿蛋白组较糖尿病正常尿蛋白组高,差异有统计学意义(P〈0.01)。结论血清CysC和PAF在早期DN患者中显著升高,提示两者在早期DN进程中起着重要作用,其测定可能有助于糖尿病及其并发症的早期诊断和病情监测。

Abstract:Objective To study the clinical value of treating the early diabetic nephropathy(DN) with cystatin C(Cys C)and platelet activating factor(PAF).Methods The expression of Cys C and PAF were detected in 30 diabetic patients with normal albuminuria,28 diabetic patients with micro-albuminuria and 15 healthy subjects.Results Cys C and PAF levels in diabetic patients with normal albuminuria[(1.42±0.41)mg/L,(67.57±19.46)μg/L] and diabetic patients with micro-albuminuria[(1.79±0.92)mg/L,(92.38±30.25)μg/L]were remarkably higher than those in normal control group[(0.74±0.21)mg/L,(50.71±15.43)μg/L](P0.01).The difference had statistical significance.Cys C and PAF levels in patients with micro-albuminuria were remarkably higher than those in patients with normal albuminuria.The difference had statistical significance(P0.01).Conclusion The remarkable increase of Cys C and PAF in patients with DN indicates that Cys C and PAF play a very important role in the early course of diabetic nephropathy.The measurement of Cys C and PAF may contribute to the early diagnosis and monitoring of diabetes and its complications.
