超导可视无痛人工流产术临床效果比较的meta分析

目的:比较超导可视无痛人工流产术与传统无痛人工流产手术的临床效果。方法:检索PubMed、EMbase、Web of Science、Cochrane Library、中国知网(CNKI)、中国生物医学文献数据库(CBM)、万方数据库(WANFANG DATA)、维普数据库(VIP),收集超导可视无痛人工流产手术临床效果及安全性的比较研究,为了减少研究间的异质性而选取近10年的研究,检索时限为2016年1-6月,采用RevMan 5.3软件进行meta分析。结果:最终纳入21篇文献,共14 482例患者。meta分析提示:超导可视无痛人工流产术的手术时间短于传统手术(P0.001),术中出血量明显少于传统手术,合并值相差4.7ml(P0.001),吸宫不全、子宫穿孔和漏吸等术中并发症的发生率均低于传统术式,人工流产综合征的发生率低于传统手术(P0.001),月经复潮时间早于传统手术(P0.001)。结论:超导可视无痛人工流产在临床上的优越性显著,改变了长期以来盲视状态下的手术操作,提高了医疗质量,值得推广应用。

Comparative clinical efficacy of painless abortion under ultrasound monitoring to conventional painless abortion: a meta-analysis

Objective:To compare the clinical effectiveness of painless abortion under ultrasound monitoring with conventional painless abortion.Methods:The relevant literatures compared ultrasound monitoring painless abortion with conventional painless abortion were searched in Databases,such as PubMed,Embase,the Cochrane Library,CNKI,CBM,VIP,and Wanfang.In order to reduce the heterogeneity of literatures,the literatures in above database were searched from 2006 to 2016,which only included literatures from nearly 10 years.RevMan 5.3 software was used for meta-analysis.Results:A total of 14452 patients in 21literatures were included.Meta-analysis showed that,compared with conventional painless abortion,surgical time of painless abortion under ultrasound monitoring was significant shorter (pooled mean difference=-1.68;95 ％ CI:-2.20 to-1.11;P =0.00),volume of bleeding was significant less (pooled mean difference=-4.68,95 ％ CI:-5.37 to 3.99,P =0.00),the incidence rate of incomplete abortion,uterus perforation and missed aspiration were significantly lower,the incidence of induced abortion syndrome was also significantly lower(P ＜0.001),time of menstruation recovery was earlier (P =0.004).Conclusion:Painless abortion under ultrasound monitoring has significant clinical advantages because of changing operation with blindsight.It can improve the service quality of abortion,and is guarantee for medical safety.So it is benefit for abortion women and is worthy to be application promotion.
