6036例腹腔镜胆囊切除术的临床分析

目的分析总结腹腔镜胆囊切除术的经验和效果。方法回顾分析6 036例患者行腹腔镜胆囊切除术的临床资料。结果 5 987例成功实施腹腔镜胆囊切除术,手术时间15~120 min,平均42 min。中转开腹49例,胆管损伤6例,腹腔出血3例,胆漏5例,脐疝1例,胆总管残余结石6例。结论合理选择适应证、熟悉常见的解剖及变异、安全有效的解剖方法及技巧是成功开展腹腔镜胆囊切除术的有效保障,及时、正确的处理并发症能最大限度降低其所导致的严重后果。

Clinical analysis of 6 036 cases of laparoscopic cholecystectomy

Objective To analyze and summarize the experience and efficacy of laparoscopic cholecystectomy（LC） . Methods The clinical data of 6036 patients underwent LC were retrospectively analyzed. Results LC was successfully completed in 5987 cases, with the operating time ranging from 15 to 120 minutes（mean,42 min）. Conversion to open surgery was required in 49 cases. Complications included 6 cases of bile duct injury, 3 cases of intraabdominal hemorrhage, 5 cases of biliary leakage, 1 case of umbilical hernia, and 6 cases of residual calculi in the common bile duct. Conclusions Appropriate choice of indication, familiality to the common anatomy and variants, and safe and effective anatomic method and technique were effective assurances for successful LC. Timely and accurate treatment of complications can attenuate serious consequence to the greatest extent.
