彩色多普勒对下肢深静脉血栓及下肢深静脉瓣功能不全的诊断价值

目的:探讨彩色多普勒对下肢深静脉血栓及下肢深静脉瓣功能不全性疾病(DVI)的诊断价值。方法:使用彩色多普勒对265例单侧或双侧下肢水肿或下肢浅静脉曲张患者髂静脉、股静脉、腘静脉及胫前、胫后等深静脉进行全面检查,分析下肢髂静脉、股静脉、腘静脉、胫前、胫后静脉二维超声图像、频谱图像、彩色多普勒血流图及血流动力学参数,并与正常对照组比较。结果:265例患者中发生深静脉血栓115例(43.3%),其中:深静脉瓣功能不全48例(18%),左下肢深静脉血栓86例(74.7%)。结论:彩色多普勒对DVI的诊断敏感性较高,方便快捷,无创伤痛苦,可重复检查,对血栓的定位、大小、栓塞的范围及病程的估计,对下肢深静脉瓣功能病变程度等方面有很大帮助。

Diagnosis of Lower Limb Venousthrombosis and Deep Venous Insufficiency with Color Doppler Flow Imaging

Objective To study the significance of color Doppler flow imaging(CDFI) on the diagnosis of lower limb deep venous thrombosis. and venous insufficiency(DVI). Methods The status of venous blood flow was examined with CDFI in 265 patients with edema of lower extremity or superficial varicosis veins of lower limb and in normal control group.The synthetic results of two-dimension ultrasound,pulsed Doppler and CDFI were dectected analyzed in iliac vein,femoral vein, poplitea vein and shin vein ,the statistical differences in index such as velocity,luminal cross,regugitant flow of the valve area between the group of the patients with lower limb deep venous thrombosis or DVI and the normal control group were observed. Results There were 115 cases with lower limb deep venous thrombosis (43.3%)and 48 cases (18%)with DVI,the DVI in left lower limb deep venous(75%) were more than that in right lower limb deep venous。Conclusion CDFI plays an important role in the diagnosis of lower limb deep venous thrombosis or DVI with higher sensitivity ,it has the characteristic of easily and repeated manipulating,non-trauma in dectecting the location,size and embolic extent,the pathogenesis extent ofvenous valve insufficiency.
