鼻内镜治疗慢性鼻-鼻窦炎疗效及影响因素分析

目的研究经鼻内镜引导下的微创手术对于慢性鼻-鼻窦炎效果的疗效,分析影响疗效的相关因素。方法采用随机数字表法选取2017年8月-2018年9月共有100例经鼻内镜引导的手术疗法的慢性鼻-鼻窦炎病人,没有合并鼻息肉的病人共有57例设为单一组,合并鼻息肉的病人共有43例设为合并组。比较两组病人在半年后的治疗效果,然后搜集病人的临床资料分析影响手术疗效的相关因素。结果单一组患者的完全控制率比合并组显著增加(P<0.05),单一组患者的未控制率比合并组显著降低(P<0.05)。经单因素分析可知影响手术疗效的因素包含患者的病型、变应性鼻炎、鼻腔粘连、Lund-Mackay评分、依从性以及复发状况;经多因素分析可知影响手术疗效的高危因素包含患者的病型、变应性鼻炎、Lund-Mackay评分、依从性以及复发状况。结论慢性鼻-鼻息肉的手术疗效影响因素较多,鼻息肉、变应性鼻炎、Lund-Mackay评分、依从性以及复发状况都能影响疗效,因此在治疗时可针对性地分析,在手术治疗的基础上矫正鼻部的结构,在术后应按时进行鼻腔清洁,避免因复发而降低治疗效果。

Endoscopic Treatment of Chronic Rhinosinusitis and Its Influencing Factors

Objective To study the effect of minimally invasive surgery guided by nasal endoscope on the effect of chronic nasal-nasal inflammation,and to analyze the related factors of the effect.Methods A total of 100 patients with chronic nasal-nose-nasal inflammation were selected in the hospital from August 2017 to September 2018,and there were 57 patients with no nasal polyps,and 43 of the patients with nasal polypus were set as the combined group.To compare the treatment effect of two groups of patients after half a year,and then to collect the clinical data of the patients and to analyze the factors affecting the operation of the operation.Results The complete control rate of the single group was significantly higher than that of the combined group(P<0.05),and the uncontrolled rate of the single group was significantly lower than that of the combined group(P<0.05).Univariate analysis showed that the factors affecting the curative effect included disease type,allergic rhinitis,nasal adhesion,Lund-Mackay score,compliance and recurrence,and multivariate analysis showed that the high risk factors affecting the curative effect included disease type,allergic rhinitis,Lund-Mackay score,compliance and recurrence.Conclusions The effect of the operation effect of the chronic nasal-nasal polyp is more,the nasal polyp,the allergic rhinitis,the Lund-Mackay score,the compliance and the relapse condition can influence the curative effect,The nasal cavity cleaning should be performed on time after the operation,and the treatment effect can be reduced due to the recurrence.
