单卵双胎妊娠和体外受精--胚胎移植4例分析

目的探讨体外受精-胚胎移植后单卵双胎的发生率、病因、诊断及防治方法及复习相关文献.方法对我院2000年1～10月接受IVF-ET治疗后妊娠病例的临床资料进行回顾性分析.结果共有751例体外受精-胚胎移植周期,330例临床妊娠,其中4例(2例为接受ICSI治疗)病人在妊娠7周超声检查发现为单卵双胎;常规IVF及ICSI后单卵双胎的发生率无显著性差别.其中2例单卵双胎合并单胎妊娠患者接受选择性减灭单卵双胎孕囊治疗.结论IVF-ET后单卵双胎的发生率增加,但其原因尚不清楚,选择性减胎术是有效的方法.要警惕单卵双胎的产科并发症.

Monozygotic Twinning and In Vitro Fertilization and Embryo Transfer:a Reprot of 4 Cases and Review of the Literature

Objectives:To study the incidence,etiology of monozygotic twinning after IVF ET;the sonographic diagnosis and the obstetrical care,which allowed the birth of health babies.Methods:A retrospective study of monozygotic twinning IVF procedures between January 2000 and October 2000. Results:A total of 751 IVF ET procedures were analysed.Three hundred and thirty clinical pregnancies were achieved in analysed group.Four cases underwent ultrasound examination at 7 weeds gestation showed monozygotic twinning.In two of those cases the embryos were obtained after ICSI.There is no significant difference between IVF and ICSI.Two cases,a triplet pregnancy occurred with MZ co existing with a singleton pregnancy were performed selective reduction.Conclusions:The incidence of monozygotic twinning is increased in group pregnancies resulting from IVF ET.No single risk factor can explain these phenomena.Selective reduction is an effective methods.This type of pregnancy needs special obstetrical attention.
