武汉市1994年孕产妇死亡及其漏报调查分析

1995年4～7月,对武汉市1994年死亡的15～49岁育龄妇女进行的全面普查。结果表明,我市孕产妇死亡率为41.16/10万,漏报率为48.28％。孕产妇死亡原因首位为产科出血,死亡专率为19.87/10万,其后依次为妊高征(7.10/10万)、妊娠合并症(5.68/10万)。孕产妇死亡与产前检查次数、死亡地点、死亡时期、分娩地点等有关。郊县孕产妇死亡漏报率为53.85％,显著高于城区。漏报环节首位为医疗保健机构,占64.29％,其次为个人家庭,占28.57%。漏报原因首位为计划外生育、隐瞒,占42.86％,其次为家属不了解报告制度。指出提高产科质量,加强围产期保健系统管理是降低孕产妇死亡率的重要措施;健全和完善医疗保健机构的孕产妇死亡报告制度,建立健全三级保健网,有效地控制计划外生育,加强在群众中宣传孕产妇死亡报告制度的重大意义,是减少和消除孕产妇死亡漏报的重要措施。

Analysis on Maternal Death and It'' s Under-reporting of Wuhan City in 1994

A complete census was undertaken on the maternal death involved with delivery aged from 15 to 49 of Wuhan city in 1994. Results: Maternal mortality rate was 41. 16/100, 000, under -reporting rate was 48. 28%. The first cause of maternal death was perinatal bleeding, which specific death rate was 19. 87/100, 000; the second was hypertensine syndrome of prenancy, Which was 7. 10/100, 000j the third was accomplicame of pvenancy, which was 5. 68/100, 000. Maternd death was involved with the times of prenatalcheck -up, time and place of death, delivery place and so on. The under - reporting rate of rnaternel mortality rate in surburb county was 53. 85% , significantly higher than it in urban. Under -repotting resulted mainly from the hospital and the family. There were several reasons of under -reporting,the fint was birth without plan and concealing,which took the percentage by 42. 86% ithe seconel was unfamiliar of reporting rule. We concluded that improving obsterical skills and tightening - up management of perinatal health care system would reduce the maternal mortality rate; perfecting the rule of maternal death reporting, establishing 3 - level health care network well, controlling the birth with plan effectively and enhancing health education about the reporting rule of maternal death among general people would reduce and eliminate the under - reporting rate.
