胸腔镜与传统开胸术治疗创伤性血气胸的疗效观察

目的 探讨采用电视胸腔镜手术与传统开胸手术治疗创伤性血气胸的临床价值.方法 选取符合标准的患者80例,采用随机数字的方法 分为观察组和对照组各40例,观察组应用胸腔镜手术,对照组应用传统开胸手术,比较二者临床效果.结果 观察组患者在手术时间、术中出血量、术后引流时间、术后引流量、术后疼痛及住院时间方面均优于对照组,差异均有统计学意义（P＜0.05）.两组患者术后第1天血清CRP水平较治疗前均升高,但是观察组升高幅度小,第3天血清CRP水平开始下降,但观察组下降幅度更大,差异均有统计学意义（P＜0.05）.观察组术后并发症发生率5%,对照组7.5%,差异无统计学意义（P＞0.05）.结论 胸腔镜手术治疗创伤性血气胸具有创伤小、术后恢复快、安全可靠等优势,值得临床推广应用.

Clinical observation of traumatic hemopneumothorax with the treatment of thoracoscopic surgery and conventional thoracotomy

Objective To discuss the clinical value of traumatic hemopneumothor with the treatment of video-assisted thoracoscopic surgery and conventional thoracotomy. Methods 80 patients were randomly divided into observation group （40 eases） and control group （40 cases）. The observation group used thoracoscopic surgery, the control group used conventional thoracotomy, the clinical effect were compared in two group. Results The operation time, intraoperative blooding, volume of drain, postoperative flow, postoperative pain and hospital stay in the observation group were better than the control group （ P 〈 0. 05）. Two group＇s serum eRP increased 1 day after operation, and the extent of variation in the observation group was more small; Two group＇s serum eRP decreased 3 day after operation, and the extent of variation in the observation group was greater, and the difference was statistically significant （P 〈 0. 05 ）. Postoperative incidence of complication was 5 % in the observation group, 7. 5 % in the control group （ P 〉 0. 05）. Conclusion Thoracoscopic surgery sould be worthy of clinical application, because it possesses the advantages of small trauma, recovery soon postoperative and safty.
