颈前路植骨内固定术治疗创伤性下颈椎失稳征

目的：评价创伤性下颈椎失稳征经颈前路减压植骨融合、带锁钢板内固定手术的治疗效果。方法：对28例确诊为创伤性下颈椎节段性不稳患者，行颈前路减压植骨融合、带锁钢板内固定术治疗，其中Orion钢板12例，AO钢板6例，Codman钢板4例，Zephir钢板6例。结果：平均随访16个月，依据Odem评分标准及截瘫指数ASIA评分标准进行手术效果评价，28例均获得骨性融合，术后椎间高度维持良好；非常满意12例(42．9％)，满意8例(28．6％)，基本满意6例(21．4％)，不满意2例(7．1％)；总有效率92．9％，优良率71．4％。结论：颈前路减压植骨融合、带锁钢板内固定手术可恢复并有效维持椎间高度，术后不稳节段恢复生理序列并获得即刻稳定，移植骨融合率高，是治疗创伤性下颈椎失稳征的良好方法。

Operative Therapy on Traumatic Lower Cervical Instability Syndrome by Anterior Decompression,Bone Graft and Inner Fixation

Objective: To investigate the efficacy of the operation on traumatic lower cervical instability syndrome by anterior decompression, bone graft and inner fixation. Methods: Diagnosed as traumatic lower cervical instability syndrome, 28 cases were treated with anterior decompression, bone graft and fixation of locking cervical plate systems by anterior approach, 12 cases with fixation of Orion Plates,6 cases by AO Plates,4 cases by Codman Plates,and 6 cases by Zephir Plates.Results: At 16 month follow-up, the operation efficacy was valued with Odom grades and paraplegia index. All the cases gained bone amalgamation and well in-tervertebral height remaining. 12 reached great satisfacting,8 felt satisfaction and 6 was basically satisfied with the operation. The effective rate was 92.9% and the good rate was 71.4% . Conclusion: By the corking operation of anterior decompression, bone graft and inner fixation on traumatic lower cervical instability syndrome, intervertebral height can be resumed and well maintained. The unstable vertebrae can obtain the'ordinary sequences, instant stability and high rate of bone fusion.
