摘    要:目的 检测老年慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者外周血环状RNA(circRNA)-HIPK3表达情况及临床意义。方法 选取南京脑科医院胸科院区2019年3月~2021年3月收治的80例老年急性加重期COPD患者(急性加重期组)及80例老年稳定期COPD患者(稳定期组)作为研究对象,另选取同期健康体检者80例作为对照组。实时荧光定量PCR(qRT-PCR)法检测外周血circRNA-HIPK3水平;采用Pearson法分析老年COPD患者外周血circRNA-HIPK3水平与肺功能指标的相关性;多因素Logisitic回归分析影响老年急性加重期COPD患者预后不良的因素;使用ROC曲线分析外周血circRNA-HIPK3水平及第1秒用力呼气容积(FEV1)对老年急性加重期COPD患者预后不良的预测价值。结果 对照组、稳定期组和急性加重期组患者FEV1(2.45±0.24L,1.69±0.35L,1.08±0.31L),FVC(2.78±0.61L,2.01±0.64L,1.47±0.46 L),FEV1/FVC(...

Abstract:Objective To detect the expression and clinical significance of peripheral blood circular RNA (circRNA)-HIPK3 in elderly patients with chronic obstructive pulmonary disease (COPD). Methods A total of 80 elderly patients with COPD in acute exacerbation stage (acute exacerbation stage group) and 80 elderly patients with stable stage COPD (stable stage group) admitted from March 2019 to March 2021 in the Thoracic Branch of Nanjing Brain Hospital were selected as the research objects, and another 80 healthy subjects during the same period were selected as the control group. Quantitative real-time PCR (qRT-PCR) method was used to detect the level of circRNA-HIPK3 in peripheral blood. Pearson method was used to analyze the correlation between the level of circRNA-HIPK3 in peripheral blood and pulmonary function indicators in elderly COPD patients. Multivariate Logisitic regression analysis was used to analyze the influence of elderly patients with acute exacerbation COPD factors of poor prognosis of patients and ROC curve was used to analyze the predictive value of peripheral blood circRNA-HIPK3 levels and FEV1 in poor prognosis of elderly patients with acute exacerbation COPD. Results Control group, stable group and acute exacerbation group FEV1(2.45±0.24L,1.69±0.35L,1.08±0.31L), FVC(2.78±0.61L,2.01±0.64L,1.47±0.46 L), FEV1/FVC(74.36%±8.02%,53.72%±9.41%,42.65%±9.22%)and DLCO%(79.36±8.92,62.58±6.91,46.39±5.31)levels were significantly lower, and the differences were statistically significant (F=409.356, 104.726,261.325,419.445,all P<0.05). The levels of circRNA-HIPK3 in the control group, stable group and acute exacerbation group (1.00±0.15, 1.74±0.46 , 2.81±0.74) were significantly increased, and the difference was statistically significant (F=254.246, P<0.05). The levels of circRNA-HIPK3 in peripheral blood of elderly COPD patients were negatively correlated with FEV1, FVC, FEV1/FVC and DLCO% (r=-0.331~-0.532, all P< 0.05). circRNA-HIPK3 (2.17±0.58 vs 3.63±0.94), FEV1(0.93±0.26L vs 1.20±0.34 L), FVC(1.33±0.41L vs 1.58±0.50 L), FEV1/FVC(35.22%±9.09 % vs 48.42% ±9.32%), DLCO% (40.88±5.17 vs 50.67 ±5.42) levels were significantly lower, and the differences were statistically significant (t=8.544,3.893,2.396,6.352,8.177,all P<0.05). Logistic regression analysis showed that circRNA-HIPK3OR (95%CI)=1.514(1.167~1.965)] was an independent risk factor for poor prognosis in elderly patients with acute exacerbation COPD (P< 0.05), while FEV1OR(95%) CI)=0.897(0.777~0.968)] was a protective factor affecting the poor prognosis of elderly patients with acute exacerbation of COPD (P<0.05). There were significant differences in the area under the curve of peripheral blood circRNA-HIPK3, FEV1 and circRNA-HIPK3 combined with FEV1 for predicting poor prognosis (AUC=0.839, 0.766, 0.947, all P< 0.05). Conclusion CircRNA-HIPK3 was highly expressed in peripheral blood of elderly COPD patients, which had a certain correlation with the lung function of patients, and has a certain predictive value for the prognosis of patients.
