8排螺旋CT多期动态增强扫描应用于肝细胞肝癌临床诊断分析

目的：探讨8排螺旋CT多期动态增强扫描在肝细胞肝癌的临床应用情况。方法：选取CT检查联合手术病理检查证实均为肝细胞肝癌的患者48例,使用8排螺旋CT进行多期动态增强扫描检查（动脉期、门静脉期、平衡期）,并对各期CT检查图像重建分析。结果：动脉期患者螺旋CT扫描显示高密度强化患者33例（68.75%）,显著高于低密度无强化患者15例（31.25%）;门静脉期患者螺旋CT扫描结果显示高密度强化患者5例（10.42%）,等密度强化12例（25.00%）,两者均显著低于低密度强化的31例（64.58%）;平衡期患者39例（81.25%）为低密度无强化,显著高于混合低密度强化的9例（18.75%）,差异均有统计学意义（P〈0.05）。结论：8排螺旋CT分期增强扫描诊断肝细胞肝癌能有效提升临床诊断效果,值得推广应用。

The Clinical Analysis of Multi-phase Dynamic 8-slices Spiral CT in the Diagnosis of Hepatocellular Carcinoma

Objective：To investigate the application value of Multi-phase dynamic 8-slices spiral CT in the diagnosis of hepatocellular carcinoma.Methods：48 patients with hepatocellular carcinoma were selected,preoperative CT scan and multi-phase enhanced scan were performed using GE 8-slices spiral CT machine,including the early arterial phase enhanced scanning,late arterial,venous phase and delayed phase.CT data were analyzed using post-processing workstation.The features of the tumors were observed through the MPR,VR and MIP.Results：The arterial phase with spiral CT scan showed a high density of 33 patients（68.75%）,significantly higher than that of low density in 15 cases（31.25%）;no enhancement in the portal venous phase with spiral CT scan results in a high density enhancement in 5 cases（10.42%）,the density enhancement in 12 cases（25%）,they were both lower than that of the low density in 31 cases（64.58%）;39 cases of patients with balance（81.25%） were of low density with no enhancement,significantly higher than that of mixed low density enhancement in 9 cases（18.75%）,the differences were statistically significant（P〈0.05）.Conclusion：Multi-phase dynamic spiral CT of hepatocellular carcinoma with high detection rates,can clearly show the characteristics of the tumor,it has high application value.
