| 金黄色葡萄球菌心内膜炎的特征及治疗 | |

| 摘 要： | 为了解金黄色葡萄球菌（金葡菌）心内膜炎的临床变迁。分析了１９７７～１９９４年我院确诊的金葡菌心内膜炎２１例，并与１９５７～１９７７年我院确诊的金葡菌心内膜炎１１例进行比较。结果显示：近年来，金葡菌心内膜炎有上升趋势，以右心内膜炎增加明显，后者尤与静脉毒瘾、血管介入性操作等有关。右心内膜炎在其高危因素、基础心脏病、临床表现及预后等方面与左心内膜炎有明显区别。室壁心内膜炎临床表现常不典型，有赖于超声心动图确诊。并发症种类有所增多且较严重。由于有效抗生素的应用，使住院病死率明显下降。提高对金葡菌心内膜炎的认识，对于早期诊断、治疗及改善预后均有重要意义。 |

| Abstract: | InordertoevaluatewhethertherewerechangesoftheclinicalfeaturesofStaphylococ-cus(Staph.)aureusendocarditisinrecentyears,thedataof21casesofStaph.aureusendocarditisdi-agnosedfrom1977to1994wereanalyzedandcomparedwiththoseof11casesofStaph.aureusendo-carditisfrom1957to1977.Theresultsshowedthefolowingchangesinrecentyears.TheincidenceofStaph.aureusendocarditiscaseshasbeenincreasing.Casesofright-sidedendocarditisincreasedintherecenttwodecadesandthisincreasewasrelatedtointravenousdrugabuseandincreaseduseofvascularintervention.Right-sidedendocarditiswasdiferentfromleft-sidedendocarditisintheirriskfactors,underlyingheartdiseases,clinicalmanifestationsandprognosis.Theclinicalmanifestationsofventricu-lar-walendocarditiswereatypicalanditcouldbedefinitelydiagnosedonlywithechocardiogram.Com-plicationsofStaph.aureusendocarditisbecamemorecommonandserious,inrecentdecadesbuthospi-talmortalitydecreasedmarkedlyduetoefectiveantibioticmanagement.TheauthorsbelievethatsoundknowledgeofStaph.aureusendocarditisisessentialfortheproperdiagnosisandtreatmentofStaph.aureusendocarditis. |
