腹腔镜阑尾切除术后窦道的预防和处理

目的通过总结该院收治的腹腔镜阑尾切除术导致窦道形成患者的治疗经历,提出窦道防治的策略。方法根据腹腔镜阑尾切除术后窦道的实际情况采取两种手术方式:方法一为窦道切口并进行炎性肉芽刮除,方法二为直接行窦道全切,比较两种手术方式的治愈效果和手术意外的可能性。结果方法一的治愈率为66.7%,方法二的治愈率为100.0%,该组病例的总治愈率为90.5%。在窦道全切过程中,有1例窦道壁破裂。结论应根据实际情况选择合适的窦道治疗方式,并做好窦道形成的预防。

Prevention and treatment of postoperative sinus of laparoscopic appendectomy

Objective To summarize the experience of prevention and treatment of sinus after laparoscopic appendectomy. Methods Operation method was decided by the sinus condition: method A, fistula incision and inflammatory granulation shaving; method B, removing he sinus completely. Then compare the efficacy and the risk of operation. Results Cure rate of method A was 66.7 %, cure rate of method B was 100.0 %. The total cure rate was 90.5 %. Sinus wall ruptured in 1 during the process of removing. Conclusions Suitable operation method should decided by the sinus condition, and make the premunition for fistula formation.
