清热祛瘀法并早期脑超声治疗脑出血临床观察

摘    要:目的:观察清热祛瘀中药联合早期脑超声治疗急性少量脑出血的疗效.方法:90例脑出血患者被随机分为治疗组60例、对照组30例.对照组采用甘露醇脱水控制血压等常规治疗,并加用清开灵注射液静脉滴注.治疗组在对照组常规治疗基础上于发病后第1~3 d开始静脉滴注脉络宁注射液20 ml,每日1~2次;在生命体征平稳的条件下,于发病第3~5 d开始使用脑超声治疗,每次30 min,每日1~2次.观察两组患者治疗20 d后血肿吸收情况和神经功能缺损评分.结果:治疗组总有效率为91.7%,对照组总有效率为73.3%,两者比较有显著性差异(P<0.01).治疗组神经功能缺损评分减少的程度明显好于对照组(P<0.05);脑出血吸收量也明显大于对照组(P<0.05).结论:应用清热、祛瘀并脑超声三联疗法治疗脑出血较单纯使用清热的清开灵注射液能明显促进血肿吸收,改善神经功能,提高临床疗效,降低致残率.

Abstract:Objective: To observe the curative effect of pyrexia and gore dispers al method combined with early cerebral ultrasonic therapy in treatment of acute cerebral hemorrhage of mild degree. Methods: Ninety cases with cerebral hemo rrhage of mild degree were randomly divided into two groups, 60 cases were a ssigned to treatment group and 30 cases to control group. The patients in cont rol group were given routine cerebral hemorrhage therapy (including control bloo d pr essure by dehydration with manitol) and intravenous Qingkailing injection. Based on the same treatment of the control group, the patients i n the t reatment group were given 20 ml Mailuoning injection intravenously , 12 times per day within 13 days after onset of cerebra l hemorrhage. For the patients with chronic subdural hematoma, the above methods were give n on the day of admission. From 3 5 days after onset of disease, while under the stable condition of life sign , th e cerebral ultrasonic therapy was used, 30 minutes each time, 12 tim es per day. After 20day treatment, the absorbing spe ed of hematoma and neurofunctional deficiency score in two groups were observed. Results: There was statistically difference in total effective rate between treatment group (917%) and control group(733%, P<001). The neurofunctional deficiency score decreased in two groups, and the change was gr e ater in the treatment group than that in the control group(P<005). Aft er treat ment, the absorbing condition of cerebral hemorrhage shown on head CT was superi or in the treatment group to control group(P<005). Conclusion: Triple combined therapy of pyrexia and gore dispersal method combined with early cerebral ultrasonic therapy can obviously promote th e absorbing of hematoma, improve nerve function, enhance the clinical curative e ffect and decrease the crippledom inducing rate.
