肾透明细胞癌的COX-2表达与MSCT征象的相关性研究

目的:探讨肾透明细胞癌(CRCC)的COX-2表达及其与MSCT征象之间的关系。方法:搜集经手术病理证实且术前有详细的MSCT资料的32例CRCC患者的石醋包埋切片及10例正常肾组织切片,采用免疫组化S-P法检测每例受检者COX-2的表达情况。观察MSCT图像上肿瘤大小,有无液化、坏死、囊变,肿瘤边界,平扫及各期增强扫描时肿瘤密度,肿瘤最大强化程度,肾静脉或/和下腔静脉有无受侵,邻近或/和远处器官有无转移等,并分析MSCT征象与COX-2表达水平的相关性。结果:CRCC组中COX-2表达明显高于正常组(P<0.01);COX-2表达与MSCT图像上肿瘤大小,最大强化程度,有无液化、坏死、囊变,肿瘤边界.肾静脉或/和下腔静脉有无受侵,邻近或/和远处器官有无转移有相关性(P<0.05);与平扫肿瘤密度无相关性(P>0.05)。结论:COX-2表达与CRCC的发生、发展有关.根据CRCC的MSCT表现在一定程度上可以判断CRCC的生物学行为和恶性程度。

Correlation between COX-2 Expression and Multi-slice Spiral CT Manifestations in Renal Clear Cell Carcinoma

Objective:To study the correlation between COX-2 expression and multi-slice spiral CT(MSCT)manifes- tations in clear renal cell carcinoma(CRCC).Methods:The MSCT images and pathology materials of 32 CRCC patients were collected.The paraffin embedded histology tissue of these 32 CRCC patients and 10 normal kidney were examined im- muno-histochemically using SP technique for COX-2 expression.CT images were evaluated for the size,margin,intra-tumor- al necrosis,liquefaction,cystic degeneration,attenuation of pre/post contrast enhancement at various phases,maximum en hancement of the tumor as well as whether renal vein,inferior vena cava,adjacent or distal organ were invaded.The correla- tion of MSCT manifestations and the level of COX-2 expression was analyzed.Results:The COX-2 expression of CRCC was significantly higher than that of normal renal tissue(P<0.01).The level of COX-2 expression was significantly correlated with the size,maximum enhancement,intra-tumoral necrosis,liquefaction,cystic degeneration and margin of the tumor,as well as invasion of renal vein or/and inferior vena cava,adjacent organs or/and distant metastases manifested on MSCT (P<0.05),but was not related to the attenuation on pre-contrast scan of the tumor(P>0.05).Conclusion:The expression of COX-2 had significant relationship with the occurrence and development of CRCC.To some extent,MSCT might predict the biological behavior and malignancy of CRCC.
