血清肌酸激酶同功酶BB和神经元特异性烯醇酶用于小细胞肺癌的临床价值

用层析荧光法和ＡＢＣ－ＥＬＩＳＡ法检测了２０例小细胞肺癌（ＳＣＬＣ），３０例非小细胞肺癌（ＮＳＣＬＣ），２５例良性肺疾病和３０例正常对照者血清中肌酸激酶同功酶ＢＢ（ＣＫ－ＢＢ）和神经元特异性烯醇酶（ＮＳＥ）。结果表明：ＳＣＬＣ组血清ＣＫ－ＢＢ和ＮＳＥ含量高于其它３组（Ｐ＜０．００１）；４组血清的ＣＫ－ＢＢ均值分别为３０．２、８．４、６．３和４．３ＩＵ／Ｌ，ＮＳＥ分别为５２．２、１２．７、１０．３、和９．２ｎｇ／ｍｌ．以９．５ＩＵ／Ｌ（ＣＫ－ＢＢ）和２０．８ｎｇ／ｍｌ（ＮＳＥ）为正常值上限，ＳＣＬＣ组的阳性率分别是７０％（ＣＫ－ＢＢ）和８０％（ＮＳＥ），高于ＮＳＣＬＣ组（Ｐ＜０．００５）。ＳＣＬＣ血清ＣＫ－ＢＢ和ＮＳＥ显著相关（ｒ＝０．７９３４，Ｐ＜０．００１），联合检测可使敏感性提高到９０％，特异性提高到９４％。因此，作者认为血清ＣＫ－ＢＢ和ＮＳＥ可以作为ＳＣＬＣ的诊断指标，联合检测可提高临床应用价值。

Clincal Value of Measurment of Serum Creatine Kinase BB and Neuron Specific Enolase for SmalI Cell Lung Cancer

Serum creatine kinase BB(CK-BB)and neuron specific enolase(NSE)were measuredwith chromatography-fluorometric method and ABC-ELISA in 20 patients with small cell Iung cancer(SCLC),30 patients with non small cell Iung cancer(NSCLC),25 patients with benign pulmonary diseases(BPD)and 30 healthy subjects(C).The results re-vealed that serum concentrations of CK-BB and NSEin SCLC were significantly greater than those in otherthree groups(P<0.001).The mean values of CK-BBand NSE in SCLC,NSCLC,BPD,Cwere30.2,8.4,6;3,4.3 IU/L and52.2,12.7,10.3,9.2 ng/ml, re-spectively,If values above 9.5IU/L(CK-BB)and20.8ng/ml(NSE)were considered abnormal,70%and 80%of the values of CK-BB and NSE in SCLCwere positive,respectively,which were higher thanthose in NSCLC(P<0. 005).The serum Ievels of CK-BB had Iinear correlatiOn with those of NSE in SCLC(r=0.7934.P<(0.001).By combined determination,the sensitivity and specificity were increased to 90%and 94%respectively,Therefore,the results suggestthat serum CK-BB and NSE be diagnostic markers forSCLC. Concurrent determination may raise their valuehdinical use.
