几丁糖关节腔注射结合手法治疗退行性膝骨关节炎临床研究

目的观察几丁糖关节腔注射结合手法治疗退行性膝骨关节炎的临床疗效。方法将120例患者随机分为对照组和治疗组,每组60例。对照组予单纯膝关节腔几丁糖注射疗法,治疗组同时加用手法治疗。两组疗程均为5周,观察临床疗效及膝关节WOMAC评分变化情况。结果治疗组、对照组总有效率分别为95.0%、88.3%,治疗组临床疗效优于对照组(P0.05)。两组治疗后膝关节疼痛、僵硬、关节功能积分均明显降低(P0.05),其中治疗组僵硬、关节功能积分降低幅度大于对照组(P0.05)。结论几丁糖关节腔注射结合手法治疗退行性膝骨关节炎的临床疗效良好,且安全可靠,相对于单纯膝关节腔注射疗法,能够更好地改善患者的膝关节僵硬及关节功能。

Clinical study on intra-articular injection of chitosan combined with manipulation in the treatment of degenerative knee osteoarthritis

  Objective:To observe the clinical efficacy of intra-articular injection of chitosan combined with manipulation in the treatment of degenerative knee osteoarthritis. Methods120 patients were randomly divided into the control group and treatment group,60 cases in each group. The control group was only treated with chitosan by intra-articular injection,the treatment group was treated with manipulation plus chitosan by intra-articular injection,with a course of 5 weeks. The clinical efficacy and the changes of WOMAC scores were observed.  Results:The total effective rates were 95.0% in the treatment group and 88.3% in the control group,and the clinical efficacy of the treatment group was better than that of the control group(P<0.05). After treatment,the scores of knee pain,stiffness and joint function were significantly decreased in both groups(P<0.05),and the reductions of stiffness and joint function scores in the treatment group were greater than those in the control group(P<0.05).  Conclusion:The intra-articular injection of chitosan combined with manipulation shows good clinical efficacy in the treatment of degenerative knee osteoarthritis,which is safe and reliable. Compared with intra-articular injection alone,it can better improve the knee stiffness and joint function of patients.
