直肠癌术后吻合口狭窄的金属内支架治疗效果评价

目的评价直肠癌术后吻合口狭窄金属内支架治疗的疗效。方法7例直肠癌术后吻合口狭窄患者，6例为吻合口肿瘤复发，1例良性狭窄。4例以急性肠梗阻就诊，3例术后排便困难就诊。采用经肛门国产镍钛合金网状支架植入治疗，观察肠梗阻解除情况及排便情况。结果7例患者支架植入操作均获成功。4例急性肠梗阻患者支架植入当时肠梗阻症状解除。3例术后排便困难明显改善。1例良性狭窄者术后2周结肠镜下将支架取出。支架植入后主要副作用为直肠刺激症状，对症处理有效。结论直肠支架植入是治疗直肠癌术后吻合口狭窄的安全有效的方法，可作为姑息性外科手术的替代方法。

The efficacy of self-expanding metallic stent implantation for treatment of postoperative colorectal anastomotic strictures

Objective To evaluate the efficiency of self-expanding metallic stent implantation for treatment of postoperative anastomotic strictures of colorectal carcinoma.Methods Seven patients with colorectal anastomotic strictures(6 cases sufferred from tumor recurrence and 1 benign stricture) were treated with metallic stent insertion.Four patients came to hospital for acute ileum,and three patients for difficult defecates.Results The procedures of stents placement were successfully performed in 7 patients.Four patients with acute ileum defecated lots of watery stool and symptomatic were relieved immediately after stent insertion.3 patients with defecated difficulty also improved significantly.In one case with benign stricture,stent was withdrawed by endoscopic after two weeks of stent placement.The main complications of stents was colorectal stimulation which could be eased by medicine.Conclusion Self-expanding metallic stent implantation is a simple and efficient method.Therefore,it might be considered as the first line approach for anastomotic strictures in some patients and could be an alternative to surgery.
