MMP9、PCNA在人肾癌中的表达及意义

摘    要:目的分析肾癌中MMF9(一种蛋白水解酶,也称明胶酶B)与PCNA(一种核内蛋白,又称周期蛋白)的表达与组织学类型、临床分期、病理分级及预后的关系。方法免疫组织化学法检测35例肾癌及12例癌旁正常肾组织中MMF9及PCNA的表达。结果MMP9在肾癌及癌旁正常肾组织中的阳性表达率有显著性差异(P=0.020),MMP9表达在肾透明细胞癌及颗粒细胞癌间无显著性差异,随肾癌分级、分期升高而明显增高,MMP9阳性表达组术后5年生存率明显低于阴性表达组;肾癌PCNA表达率高于癌旁正常肾组织(P=0.008),肾癌的PCNA表达率随肿瘤分级增高而增高,各分期间差异无显著性(H=5.746,P〉0.05)。肾癌的PCNA表达与预后有关,与肾癌的组织类型无关(P=0.329)。结论MMF9及PCNA的检测可作为肾癌的辅助诊断及恶性程度和预后的参考指标。

Abstract:ObjectiveTo study the expression of MMP9 and PCNA in renal cell carcinoma(RCC) and its relation to the RCC histological types,clinical stages,and prognosis,respectively.MethodsMMP9 and PCNA expression were immunohistochemically detected in 35 RCC cases and 12 cases of normal renal tissues adjacent to neoplastic lesions.ResultsThe positive rate of MMP9 was higher in tumor tissue than that in nonneoplastic tissue(P=0.020),and correlated with the stages,grades,and prognosis of the patients,there was no significant difference between clear RCC and granular cell carcinoma.The positive rate of PCNA in tumor tissue was higher than that in normal renal tissues adjacent to neoplastic lesions(P=0.008),and was related to the pathological grades and the prognosis of the patients.There exist no significant differences of the positive rate of PCNA between the pathological stages(H=5.746,P>0.05) of RCC or the histological types(P=0.329).ConclusionMMP9 and PCNA could be used as predictors for supplemental diagnosis,evaluation of the malignancy and prognosis of RCC.
