我院2008年上半年门诊处方用药分析

目的：通过调查门诊的用药现状,加强处方审核,提高门诊处方质量,促进合理用药,保障患者用药安全有效。方法：随机抽取2008年1月至6月门诊处方（麻醉药品、精神药品、中草药处方除外）,共计2400张,将书写不规范的处方及不合理用药的处方进行归类分析。结果：抽取的2400张处方中,不合格处方为668张。其中书写格式不规范处方572张,占抽样处方的23．83％,主要表现为使用英文名称和商品名、前记缺项等;不合理用药处方96张,占抽样处方的4％,主要表现为联用不合理。结论：处方书写欠规范,合理用药有待提高。药师应认真肩负起审核处方的职责,加强与临床医生的合作交流,为促进合理用药发挥应有的作用。

The Analysis of Clinic Prescription of Xiangzhou Hospital in Zhuhai City During the First Half Year in 2008

Objective ：To improve the quality of prescription in clinic, keeping the rational and safety drug use, we made the investigating and verify of prescription in clinic from January to June 2008. Method： We took the clinic prescription at random from January to June 2008 （ except the anesthetic drugs, Psychotropic and Chinese herb）. And took the grouping analysis of these prescriptions by the standard form or irrational use of drugs. Result： There are 668 pieces of unqualified prescription from the 2400 prescription totally. As the unqualified prescriptions, there are 572 pieces of prescriptions （23.83%） because of nonstandard form, such as writing the drug in English or trade name. And there are 96 pieces of prescriptions （4%） for the irrational use of drugs, such as irrational combine use of drugs. Conclusion： There are some problems of the clinic prescription. The situation of the prescription should be improved, such as rational use of drugs or the nonstandard form of the prescriptions. Pharmacist should improve the communication with clinic doctors. And take the more responsibility to improve situation of the rational use of drugs.
