抗心磷脂抗体检测及其临床意义

本文报告用ELISA法检测了SLE及其它结缔组织病病人的抗心磷脂抗体(ACA)。53/110例SLE、7/9例SS、3/10例PSS和178例PM及DM病人ACA阳性。ACA的检测有助于上述疾病的诊断。ACA阳性SLE病人与ACA阴性SLE病人比较,网状青斑(31.4%比8.6%)、皮肤血管炎(68.6%比40%)、肺脏受累(22.9%比8.7%)、血小板减少(68.6%比37.1%)和抗Ro/SSA抗体阳性(57.9%比30.3%)发生率高。5例有血栓形成的病人中,1例有ACA。本组其它结缔组织病中,ACA阳性病人多数表现皮肤血管炎和高r球蛋白血症。

Anticardiolipin Antibodies in Patients with Systemic Lupus Erythematosus and Other Connective tissue Diseases.

IgG—ACA were detected in patients with SLE and other connective tissue diseases. ACAwas positive in 53 of 110(48.2%) cases with SLE,3 OF 10(30%) cases with PSS.7 of 9(77.8%) cases with Sjogren's syndrome ,and 4 of 8(50.0%)cases with PM/DM.Comparing the clinical and serological abnormalities of ACA positive SLE with ACA negative SLE, ACA positive group was found to have higher frequency of livido reticularis (31.4% vs 8.6%),cutaneous vasculitia (68.6% va 40.0%), pulmonary involvement (22.9% vs 5.7% ) ,thrombocytopenia (68. 6% vs 37. 1%) and anti — Ro/SSA antibody (57.9% vs 30.0%). Two cases of SLE with muhipie thrombosis including arterial and venous thrombosis ,Pulmonary embolus and thrombosis of arteria of retina were both ACA positive. Cases of other CTDS with ACA were found to have higher prevalence of cutaneous vasculitis and hypergammaglobulinemia.
