YLD-200输卵管通液治疗仪行输卵管造影和再通术的临床应用评价

摘    要:目的对YLD-200榆卵管通液治疗仪进行输卵管造影和再通术的再通原理、安全性和有效性进行评价。方法收集2000年1月~2012年6月门诊、住院女性不孕患者500例,在X线监视系统下,用YLD-200输卵管通液治疗仪,利用增大造影剂流体压力原理,使造影剂对输卵管形成射流、扩张、分离,达到输卵管阻塞再通目的。结果纳入女性不孕患者500例,年龄20~38岁,平均25岁。输卵管通畅152例,输卵管阻塞348例。输卵管阻塞再通成功261例,再通率75%(261/348)。输卵管再通261例随访,术后3~6月怀孕52例,6~12月71例,1年后怀孕39例,怀孕率62%(162/261)。术中轻微腹痛368例,能忍耐,未予特殊处理。中等程度腹痛132例,经减压等对症处理后均可减轻。上述患者腹痛症状均于术后1~3小时缓解。未观察到严重并发症发生。结论YLD-200输卵管通液治疗仪进行输卵管造影和再通,诊断造影压力控制在8~12Kpa,再通治疗压力控制在13~17Kpa为宜。该方法对输卵管阻塞诊断直观、确切,输卵管阻塞再通率高,效果好,无严重并发症发生,安全可靠,值得临床推广应用。

Value of type YLD-200 fluid-inject machine in hysterosalpingography and fallopian tube recanalization

Abstract:Objective To explore the value of type YLD-200 fluid-inject machine in diagnosis and treatment in infertile women with fallopian tube obstruction or stenosis. Methods All infertile women were enrolled in this study of their own accord from January 2000 to June 2012 in our hospital. These enrolled women were treated by the type YLD- 200 fluid-inject machine to visualize the fallopian tubes and dilate them if stenosis or obstruction exist for fallopian tube recanalization. Results Five hundred infertile women were enrolled in this study. Median age was 25 years (range, 20-38years). Among them, 34:8 cases were found stenosis or obstruction in fallopian tubes, 152 had normal fallopian tubes. 261 patients with fallopian tubes stenosis or obstruction had got reeanalization after treatment (75%) and 162 cases of them had conceived successfully (62%). 52 cases were in 6 months, 71 cases were between 6 and 12 months, 39 cases were after 1 year. As to adverse events, 368 cases patients felt mild abdomen pain and 132 cases suffered moderate abdomen dull pain during treatment. All the pain returned to normal in 3 hours after symptomatic treatment, no severe adverse event was observed. Conclusion Our results show that type YLD-200 fluid-inject machine is safe and effective in diagnosis and treatment in infertile women with fallopian tube obstruction or stenosis. Meanwhile, the best pressure for diagnosis is between 8-12Kpa and fallopian tube dilation was 13-17Kpa.
