经翼点入路大脑前动脉A1段和前交通动脉及其穿通支的显微解剖

目的观察大脑前动脉（ACA）A1段和前交通动脉（AcoA）及其穿支数目、走行、分布区域。方法10例成人尸体头颅,经翼点入路开颅,手术显微镜下观察ACA的A1段和AcoA发出穿支的部位、走行、数目及分布区域。结果翼点入路可以很好暴露ACA的A1段和AcoA及其穿支。A1穿支包括回返动脉、前穿动脉、丘脑下部穿通支。AcoA穿支均起自后壁和上壁,平均3．2支,止于下丘脑前部视交叉背面和前穿质。结论经翼点入路观察到了ACA的A1段和AcoA及其穿支的解剖结构。

Microsurgical anatomy on A1 segment of anterior cerebral artery and anterior communieating artery and their perforating arteries through pterional approach

Objecttive To study the microanatomy of A1 segment of anterior cerebral artery（ACA） and anterior communieating artery（AcoA） and their perforating arteries. Methods Microanatomy anatomy and measurement about A1 segment of ACA and AcoA and their perforating arteries were performed in 10 adult heads along with the path of the pterional approach under the surgical microscope. Results The pterional approach could provide excellent exposure of A1 segment of ACA and AcoA and their perforating arteries. The perforating arteries of A1 include Heubner＇s recurrent artery,antero-perforante arteries, hypothalamus perforating arteries. All of the perforating arteries of AcoA arising from the posterior and inferior wall of A1, the average number of 3.2, stop at the front of the hypothalamus, the dorsal aspect of optic chiasma and anterior perforated substance. Conclusion The microanatomy of A1 segment of ACA and AcoA and their perforating arteries are observed through peterional approach.
