肝功能异常与妊娠并发症的关系及对围生儿的影响

目的探讨孕期肝功能异常与妊娠并发症的关系及对围生儿的影响。方法回顾性分析2006年1月至2010年11月住院分娩的肝功能异常孕妇142例,选择其中62例谷丙转氨酶（ALT）≥正常上限值（ULN）2倍作观察组,余80作对照组,两组病例就妊娠并发症、围生儿情况进行比较。结果观察组中妊娠高血压疾病（PIH）、产后出血的发生率均高于对照组,差异有统计学意义,其胎儿窘迫、羊水粪染、胎儿生长受限（FGR）的发生率明显高于对照组。肝功能异常伴妊娠并发症患者胎儿窘迫、羊水粪染、FGR远较无妊娠并发症患者高。新生儿窒息的发生率无统计学意义。结论孕期肝功能异常与妊娠并发症及围生儿有着密切的关系,并且损害程度越重,对母婴生命造成威胁越大,加强孕期肝功能的检测并积极治疗能有效减少妊娠并发症的发生及对围生儿的影响。

Abnormal liver function and relationship with pregnancy complications or influence of perinatals

Objective To explore the relatioship of abnormal liver function and pregnancy complications and its influence on perinatals.Methods 142 patients with abnormal liver function from January 2006 to November 2010 were retrospectively anayzed,62 cases with alanine aminotransferase 2 times higher than nomal were chosen as observation group,the rest as control group.Then compare the pregnancy complications and the influence on perinatals between the two groups.Results The incidence of PIH、ostpartum hemorrhage in observation group higher than control group,the difference was meaningful in statistical,and the incidence of fetal distress、amniotic fluid dung dye、FGR higher than control group too.Abnormal liver function with pregnancy complications in pregnant women,its fetal distress、amniotic fluid dung dye、FGR far higher than those without pregnancy complications.the incidence of neonatal asphyxia was meaningless in statistical.Conclusion Here are closely relationship with abnormal liver function during pregnancy and pregnancy complications or influence of perinatals.and more damage degree,the more harmful to maternal-fetal.Strengthen liver function test during pregnancy and actively treatment can reduce pregnancy complications and influence of perinatals effectively.
