类风湿关节炎患者抑郁焦虑情绪及其相关因素的调查分析

目的调查类风湿关节炎（Rheumatoid Arthritis,RA）患者抑郁焦虑情绪发生率及其相关因素.方法采用Zung抑郁自评量表（SDS）及焦虑自评量表（SAS）和肖水源的社会支持量表调查58例RA患者,以压痛关节数多少分组,进行对照比较.结果类风湿关节炎患者抑郁、焦虑情绪发生率分别为46.55%（27/58）和39.66%（23/58）.其中抑郁症状以晨重晚轻、能力减退、易激怒、疲乏、性兴趣减退列前五位.而焦虑以静坐不能、乏力、睡眠障碍、不幸预感、多汗等症状常见.抑郁、焦虑情绪与RA患者关节压痛数（P＜0.01）、关节肿胀数（P＜0.01）呈正相关,与握力（P＜0.01）呈负相关,与晨僵、年龄、病程、社会支持（P＞0.05）无明显相关性.结论RA患者存在焦虑、抑郁情绪,焦虑、抑郁与患者的关节压痛数、关节肿胀数、握力有关.

Investigation on Anxiety, Depressive Mood and Relative Factors of Patients with Rheumatoid Arthritis

Objective To investigate the incidence of anxiety and depressive mood in patients with rheumatoid arthritis(RA) and relative factors.Methods 58 patients with RA were investigated by(self-rating) anxiety scale(SAS),self-rating depression scale(SDS) and social support questionnaire(by Xiao Shuiyuan).The patients were divided into two groups according to joint tenderness count.(Results The) incidence of anxiety and depressive mood in RA was 46.55%(27/58) and 39.66%(23/58) respectively.High frequency of depressive mood included diurnal variation of mood,incompetent,enrage,tired,loss of libido and so on.Anxiety mood included acathisia,fatigue,difficult to sleep,premonition of unpleasant event,sweating and so on.The anxiety and depressive mood in patients with RA were positively correlated with joint tenderness count(P<0.01),joint swelling count(P<0.01),and negatively correlated with grip strength(P<0.01).It was not obviously correlated with duration of morning stiffness,age,disease course,social support(P>0.05).Conclusions Patients with RA have anxiety and depressive mood.Anxiety and depression mood are associated with joint tenderness count,joint swelling count and grip strength.
